目的:探讨禄丰县新型农村合作医疗制度补偿方案的实践性与可行性,解决实践中出现的问题,提高农民受益程度,以资在云南省范围内全面实施新农合制度时参考。方法:采用现场流行病学研究方法进行研究。结果:禄丰县新农合制度补偿方案是“门诊按比例减免+住院费用按比例补偿”模式。2004年度,该县对门诊减免和住院补偿办法做了较大幅度调整。两个乡(镇)参合农民、医务人员与参合农民对新农合制度补偿方案满意度评价分布的差异分别有统计学意义(P<0.001)。结论:禄丰县现行新农合制度补偿度补偿方案基本可行,社会满意度较高,值得借鉴和推广,建议调整门诊减免办法,单设门诊减免封顶线,提高乡(镇)卫生机构住院补偿比例。
Objective: To explore the practicality and feasibility of the compensation scheme for the new rural cooperative medical system in Lufeng County, to solve the problems in practice, and to increase the level of beneficiaries of farmers, so as to make reference when fully implementing the new rural cooperative medical system in Yunnan Province. Methods: Field epidemiological research methods were used for research. Results: The compensation scheme for the new rural cooperative medical system in Lufeng County is a model of “proportionate exemption of outpatients + proportional compensation for hospitalization”. In 2004, the county made substantial adjustments to outpatient reduction and hospitalization compensation measures. There was a statistically significant difference in the evaluation of the satisfaction of the compensation schemes of the two rural (town) participating farmers, medical staff and participating farmers in the compensation scheme for the new rural cooperative system (P<0.001). Conclusion: The current compensation rate compensation program for the new rural cooperative medical system in Lufeng County is basically feasible and social satisfaction is high. It is worth learning from and popularizing. It is recommended to adjust the outpatient discounting measures, set up the ceiling line for outpatient relief, and increase the proportion of hospitalization compensation for township (town) health institutions. .
