目的探讨膀胱癌组织中CD4~+、CD8~+及CD20~+淋巴细胞浸润情况及其与膀胱癌病理分级和临床分期的关系。方法应用免疫组织化学二步法检测34例膀胱癌患者组织中CD4、CD8及CD20分子的表达,并分析其与临床病理特征的关系。结果 34例膀胱癌组织中,CD4、CD8和CD20染色阳性率分别为100.0%、85.3%和64.7%,CD4、CD8阳性率均明显高于CD20(P<0.05)。CD4分子在所有膀胱癌组织标本中均呈阳性表达;CD8分子在不同病理分级和临床分期组的表达差异均无统计学意义(P>0.05),但其阳性率随病理分级及临床分期的增加有下降的趋势;CD20分子在不同病理分级组中的表达差异无统计学意义(P>0.05),其阳性率随临床分期增加有升高的趋势,但差异无统计学意义(P>0.05)。结论膀胱癌组织以T淋巴细胞浸润为主,且CD8~+T细胞可能在其中发挥更为积极的作用,炎症细胞浸润可能是膀胱癌变过程中的早期事件。
Objective To investigate the infiltration of CD4 ~ +, CD8 ~ + and CD20 ~ + lymphocytes in bladder cancer and its relationship with pathological grading and clinical stage of bladder cancer. Methods The expressions of CD4, CD8 and CD20 in 34 cases of bladder cancer tissues were detected by immunohistochemical two-step method and their relationship with clinicopathological features was analyzed. Results The positive rates of CD4, CD8 and CD20 in 34 cases of bladder cancer were 100.0%, 85.3% and 64.7%, respectively. The positive rates of CD4 and CD8 were significantly higher than those of CD20 (P <0.05). CD4 was positively expressed in all specimens of bladder cancer. There was no significant difference in the expression of CD8 between different pathological grade and clinical stage (P> 0.05), but the positive rate of CD8 increased with the pathological grade and clinical stage (P> 0.05). The positive rate of CD20 increased with clinical stages, but the difference was not statistically significant (P> 0.05) . Conclusions T lymphocyte infiltration is the most common in bladder cancer tissues, and CD8 + T cells may play a more active role in this process. Inflammatory cell infiltration may be an early event in bladder carcinogenesis.
