二氧化碳激光对扁桃体组织损伤的研究

目的观察不同功率二氧化碳(carbon dioxide,CO2)激光对扁桃体组织的损伤情况,确定CO2激光在扁桃体切除术中的最佳能量范围。方法采用传统方法摘除的扁桃体组织50侧,在相同照射时间条件(4s)下,以不同输出功率(1.0、2.0、3.0、4.0 W)的CO2激光(光斑直径为0.2 mm)对同一扁桃体进行定点照射。照射后即刻制备组织切片,光学显微镜下进行观察。结果功率密度为0.4 W/m2[(0.000 50±0.000 04)mm3]和0.8 W/m2[(0.000 81±0.000 03)mm3]两组间、0.8 W/m2和1.2 W/m2[(0.001 25±0.000 02)mm3]两组间,热损伤容积差异均无统计学意义(P>0.05);功率密度为0.4 W/m2和1.2 W/m2两组间、0.4 W/m2和1.6 W/m2[(0.003 72±0.000 05)mm3]两组间、0.8 W/m2和1.6 W/m2两组间、1.2 W/m2和1.6 W/m2两组间热损伤容积差异均有统计学意义(P<0.01)。功率密度与热损伤容积相关系数为0.791。结论CO2激光扁桃体切除术的最佳能量范围为1~3 W,应根据患者年龄及扁桃体局部炎性粘连程度适当选择。 Objective To observe the damage of tonsil tissue with different carbon dioxide (CO2) laser and determine the best energy range of CO2 laser in tonsillectomy. Methods 50 tonsils of tonsil tissue were removed by traditional method. The same tonsil was fixed with CO2 laser with different output (1.0, 2.0, 3.0 and 4.0 W) under the same irradiation time (4 s) Irradiation. Tissue sections were prepared immediately after irradiation and observed under a light microscope. Results Power densities were 0.4 W / m2 [(0.000 50 ± 0.000 04) mm3] and 0.8 W / m2 [(0.000 81 ± 0.000 03) mm3] ± 0.000 02) mm3]. There was no significant difference in the volume of thermal damage between the two groups (P> 0.05). The power density was 0.4 W / m2 and 1.2 W / m2 in the two groups, 0.4 W / m2 and 1.6 W / m2 [(0.003 72 ± 0.000 05) mm3] There were significant differences in the volume of thermal injury between the two groups at 1.2 W / m2 and 1.6 W / m2 at 0.8 W / m2 and 1.6 W / m2 <0.01). The correlation coefficient between power density and thermal damage volume was 0.791. Conclusion CO2 laser tonsillectomy the best energy range of 1 ~ 3 W, should be based on the age of patients and tonsil partial inflammatory adhesions appropriate choice.
