目的探讨辅助性T细胞17(Th17)表达水平与儿童哮喘的关系,为哮喘的临床治疗提供参考依据。方法采用以医院为基础病例对照研究方法对2014年1—12月重庆市中医院呼吸内科收治的150例哮喘患儿及同期在该医院进行体检的200名健康儿童进行肺功能、外周血Th17细胞及白细胞介素-4、白细胞介素-17、白细胞介素-8、干扰素-γ等细胞因子检测,分析Th17细胞水平与各细胞因子的相关性。结果病例组哮喘患儿的第1秒用力呼气容积占预计值的百分比和最大呼气峰流速占预计值的百分率分别为(68.3±5.9)%和(70.4±6.2)%,均低于对照组健康儿童的(87.9±6.1)%和(92.4±6.7)%(均P<0.001);病例组哮喘患儿的外周血Th17细胞、白细胞介素-4、白细胞介素-17和白细胞介素-8水平分别为(2.48±0.87)%、(166.84±29.45)ng/L、(5.30±2.14)ng/L和(529.46±193.20)pg/m L,均高于对照组健康儿童的(0.26±0.12)%、(68.95±12.37)ng/L、(2.89±1.70)ng/L和(157.97±48.24)pg/m L,干扰素-γ水平为(35.16±5.04)ng/L,低于对照组健康儿童的(51.49±7.02)ng/L(均P<0.001);相关分析结果显示,哮喘患儿外周血Th17细胞水平与IL-17水平呈正相关(r=0.437,P<0.001)。结论 Th17细胞在哮喘儿童外周血的表达水平高于健康儿童,其作用可能与通过IL-17介导炎症反应的发生有关。
Objective To investigate the relationship between the expression of helper T cell 17 (Th17) and asthma in children and to provide a reference for the clinical treatment of asthma. Methods A case-control study based on a hospital-based method was used to evaluate the pulmonary function, peripheral blood Th17 cells in 150 asthmatic children admitted to Department of Respiratory Medicine, Chongqing Hospital of Traditional Chinese Medicine from January to December 2014, and 200 healthy children undergoing physical examinations at the same hospital. And interleukin-4, interleukin-17, interleukin-8, interferon-γ and other cytokines were detected and the correlation between Th17 cell level and various cytokines was analyzed. Results The percentage of forced expiratory volume in 1 second and the predicted maximum expiratory flow in children with asthma were (68.3 ± 5.9) and (70.4 ± 6.2)%, respectively, which were lower than those of control The levels of Th17 cells, interleukin-4, interleukin-17 and interleukin-1 in peripheral blood of children with asthma were (87.9 ± 6.1)% and (92.4 ± 6.7)% -8 levels were (2.48 ± 0.87)%, (166.84 ± 29.45) ng / L, (5.30 ± 2.14) ng / L and (529.46 ± 193.20) pg / m L, respectively, which were higher than those in healthy children ± 0.12)%, (68.95 ± 12.37) ng / L, (2.89 ± 1.70) ng / L, and (157.97 ± 48.24) pg / m L respectively.
