目的:研究凋亡相关蛋白Bcl-2、Bax、Fas和Fas-L在非霍奇金淋巴瘤(NHL)中的表达及意义。方法:应用免疫组化S-P法对54例NHL进行检测。结果:(1)Bcl-2、Bax、Fas和Fas-L阳性率分别为50%、83.8%、88.9%和88.7%。(2)反应性增生淋巴滤泡与肿瘤性滤泡的Bcl-2表达方式相反。(3)Bcl-2在B细胞性NHL中的表达高于T细胞性NHL。(4)低度恶性B细胞性NHL中Bcl-2的表达高于中~高度恶性组,而Fas-L的表达强度低于中~高度恶性组。结论:Bcl-2的免疫组化检测是鉴别淋巴结反应性增生与滤泡性淋巴瘤较可靠的诊断方法。Bcl-2在B细胞性NHL发病机制中比在T细胞性NHL中可能发挥更重要的作用。对Bcl-2和Fas-L的免疫组化检测可能对估计NHL恶性程度及预后有一定价值。
Objective: To study the expression and significance of apoptosis-related proteins Bcl-2, Bax, Fas and Fas-L in non-Hodgkin’s lymphoma (NHL). Methods: 54 cases of NHL were detected by immunohistochemical S-P method. Results: (1) The positive rates of Bcl-2, Bax, Fas, and Fas-L were 50%, 83.8%, 88.9%, and 88.7%, respectively. (2) The Bcl-2 expression patterns of reactive proliferative lymphoid follicles and neoplastic follicles are opposite. (3) The expression of Bcl-2 in B-cell NHL was higher than that in T-cell NHL. (4) The expression of Bcl-2 in low-grade B-cell NHL was higher than that in the medium-high-grade group. The expression level of Fas-L was lower than that in the medium to high-grade group. Conclusion: Immunohistochemical detection of Bcl-2 is a reliable method for differential diagnosis of lymph node reactive hyperplasia and follicular lymphoma. Bcl-2 may play a more important role in the pathogenesis of B-cell NHL than in T-cell NHL. Immunohistochemical detection of Bcl-2 and Fas-L may have some value in estimating the malignancy and prognosis of NHL.
