In insulin-dependent diabetes mellitus (IDDM), the pancreas’s beta cells are disrupted by the immune-mediated process and therefore a large number of functional beta cells must be lost before symptoms of hyperglycemia develop. Normally, this beta cell destruction takes several years, during which time it is completely asymptomatic and its blood sugar is normal. The main focus of the debate is whether, once activated, immune-mediated beta-cell disruption manifests itself as harsh, progressive, and ultimately clinical IDDM; or the disease process can be either enlarged or diminished, sometimes temporarily or persistently Ease.
