1例57岁肝炎后肝硬化男性患者因腹腔感染给予头孢美唑钠2g溶于5%葡萄糖100ml中静脉滴注。滴注结束后约5min,患者突然出现寒颤、胸闷。查体:P130次/min,R24次/min,BP130/70mmHg,血氧饱和度0.95,双肺呼吸音粗。立即给予吸氧,静脉给予地塞米松2mg和10%葡萄糖酸钙。40min后患者症状缓解。2.5h后,患者P96次/min,R22次/min,BP110/70mmHg。追踪病史示患者在1个月前曾静脉滴注头孢美唑钠2g,2次/d,共15d,未见不良反应。
A 57-year-old male patient with posthepatitic cirrhosis was given intravenous infusion of cefmetazole sodium 2 g in 100 ml of 5% glucose for intraperitoneal infection. About 5 minutes after the end of instillation, the patient suddenly appeared shivering, chest tightness. Examination: P130 times / min, R24 times / min, BP130 / 70mmHg, oxygen saturation of 0.95, lung breath sounds coarse. Immediately given oxygen, intravenous dexamethasone 2mg and 10% calcium gluconate. Symptoms were relieved after 40 minutes. After 2.5h, the patients were treated with P96 / min, R22 times / min and BP110 / 70mmHg. Tracing the history of patients showed that patients had cefmetazole sodium 2g 2 times a day, 2 times / d, a total of 15 days, no adverse reactions.
