目的研究孝感地区丙型肝炎病毒(HCV)基因分型特点及临床意义。方法收集2011年6月-2015年6月孝感地区398例HCV患者的临床资料和外周静脉血,采用RT-PCR法测定HCVRNA含量,采用sanger测序法检测HCV基因型,用双试剂速率法检测肝功能指标ALT、AST,对所测数据进行统计分析。结果孝感地区患者HCV存在7种基因型,以1b型最多占71.11%,其次2a型占16.33%,HCV、HBV重叠感染中HCV基因型以基因型1b2a为主,其所占比率比单纯HCV感染要高,差异有统计学意义(P<0.01),HCV感染患者中男性感染1b型的比率高于女性,女性感染2a型的比率高于男性,差异有统计学意义(P<0.01);HCV感染患者年龄段主要分布在30~60岁,不同年龄段均以感染HCV1b型为主,HCV的不同传播途径中经血液透析、母婴垂直传播为1b和2a型,经输血和性传播的有6种基因型,静脉吸毒、其他途径、不明原因传播的有7种基因型,呈散在分布;HCV1型感染患者血清HCV-RNA载量比2、3、6型要高(P<0.01),HCV1型感染患者血浆ALT含量比2型、3型、6型血浆ALT含量高(P<0.01)。结论孝感地区患者HCV基因分型呈分散多样化,以1b型为主,HCV基因分型、病毒载量、传染途径以及对肝脏的损伤程度对临床诊疗有重要价值,为本地区慢性丙肝患者评估病情、制订个体化治疗方案提供依据。
Objective To study the genotyping and clinical significance of hepatitis C virus (HCV) in Xiaogan area. Methods The clinical data and peripheral venous blood samples from 398 patients with HCV in Xiaogan area from June 2011 to June 2015 were collected. The HCV RNA content was determined by RT-PCR, the HCV genotype was detected by Sanger sequencing, Function indicators ALT, AST, the measured data for statistical analysis. Results There were 7 genotypes of HCV in Xiaogan region, accounting for 71.11% in type 1b and 16.33% in type 2a. HCV genotypes were predominantly genotype 1b2a in HCV and HBV infection, which was higher than HCV infection (P <0.01). The prevalence of genotype 1b was higher in women with HCV infection than in women, and the prevalence of genotype 2a in women with HCV infection was significantly higher than that in men (P <0.01). HCV Infected patients mainly in the age group 30 to 60 years of age, all infected with HCV1b-based at different ages, HCV in different transmission routes by hemodialysis, mother and baby vertical transmission of 1b and 2a type, transfusions and sexually transmitted There were 7 genotypes in 6 genotypes, intravenous drug use, other ways and unknown reasons. The HCV-RNA load of patients with HCV1 infection was higher than that of 2, 3 and 6 (P <0.01) Patients with HCV1 infection had higher plasma levels of ALT than those with type 2, 3 and 6 plasma (P <0.01). Conclusion The genotypes of HCV in Xiaogan area are scattered and diversified. The main genotype 1b is genotype 1b. HCV genotyping, viral load, route of infection and liver damage are of great value in clinical diagnosis and treatment. Condition, to develop individualized treatment programs provide the basis.
