CD147与MMP-2在葡萄胎组织中的表达及其意义

目的:探讨CD147、MMP-2在葡萄胎组织中的表达及两者间的相互联系,预测葡萄胎恶变的可能。方法:应用免疫组织化学法对葡萄胎患者首次清宫术后40例组织标本(其中葡萄胎组31例,侵葡组6例,失访3例)进行CD147、MMP-2的检测,并与正常早孕患者绒毛组织作比较。再将葡萄胎高危组分为恶变组和未恶变组,应用免疫组织化学法检测CD147、MMP-2在含有高危因素的葡萄胎患者中的表达,分析其与葡萄胎组织恶变的关系。结果:①CD147主要表达于合体滋养层细胞以及增生滋养层细胞的胞膜,少量表达于胞浆;MMP-2主要表达于合体滋养层细胞以及增生滋养层细胞的胞浆,少量表达于胞膜。②CD147、MMP-2在侵蚀性葡萄胎组、葡萄胎组、正常绒毛组的表达呈依次递减的趋势,且两组间比较有统计学意义。③在含有高危因素的恶变组中,CD147、MMP-2平均灰度值较未恶变组的平均灰度值低(P<0.05)。④侵蚀性葡萄胎组、葡萄胎组、正常绒毛组中MMP-2与CD147的表达呈正相关。结论:①CD147、MMP-2在葡萄胎、侵蚀性葡萄胎发生发展过程中可能有促进作用,且有可能促进葡萄胎组织的恶变。②在含有高危因素的葡萄胎恶变患者中CD147、MMP-2的表达较未恶变组明显升高,提示联合检测CD147、MMP-2对预测葡萄胎的恶变具有一定的临床意义,有可能成为临床预测葡萄胎恶变的重要指标。 Objective: To investigate the expression of CD147 and MMP-2 in hydatidiform mole and their correlation with each other to predict the possibility of hydatidiform mole malignancy. Methods: Immunohistochemical method was used to detect the expression of CD147 and MMP-2 in 40 cases of hydatidiform mole patients after the first curettage (including 31 cases of hydatidiform mole, 6 cases of invasive group and 3 cases of lost-treated group) Villus tissue of pregnant women for comparison. The high-risk mole of hydatidiform mole was further divided into malignant group and non-malignant group. Immunohistochemistry was used to detect the expression of CD147 and MMP-2 in patients with hydatidiform mole with high risk and the relationship between malignant transformation and hydatidiform mole. Results: ①CD147 was mainly expressed in the membrane of syncytiotrophoblast and hyperplastic trophoblast cells, and a small amount was expressed in the cytoplasm. MMP-2 was mainly expressed in the cytoplasm of syncytiotrophoblast and hyperplastic trophoblast cells, and a small amount was expressed on the cell membrane. ② The expression of CD147 and MMP-2 in descending hydatidiform mole, hydatidiform mole group and normal villus group showed a decreasing trend in turn, and the two groups were statistically significant. ③In malignant group with high risk factors, the average gray value of CD147 and MMP-2 was lower than that of non-malignant group (P <0.05). ④ The expressions of MMP-2 and CD147 in aggressive mole group, hydatidiform mole group and normal villus group were positively correlated. Conclusion: ①CD147 and MMP-2 may promote the development of hydatidiform mole and invasive hydatidiform mole, and may promote the malignant transformation of hydatidiform mole. ②The expression of CD147 and MMP-2 in patients with hydatidiform mole with high risk factor was significantly higher than that in non-malignant group, suggesting that the combined detection of CD147 and MMP-2 has some clinical significance in predicting the malignancy of hydatidiform mole and may become a clinical Predict malignant mole of an important indicator.
