目的探讨慢性乙肝合并糖尿病患者不同血清乙型肝炎病毒核糖核酸(HBV-DNA)载量与空腹血糖(FPG)、糖化血红蛋白(Hb A1c)的相关性。方法 2011年11月-2015年10月选择河北省唐山市传染病院收治的慢乙肝合并糖尿病患者115例为研究对象,同时选择同期25例健康体检者作为对照组。入院后第2 d清晨抽空腹静脉血,测定空腹血糖(FBS)和糖化血红蛋白(Hb Alc),并采用荧光定量PCR法进行检测患者血清中HBV-DNA含量,按照血清HBV-DNA载量将患者分为4组,并分析HBV-DNA含量与FBS、Hb Alc相关性。结果所有患者血清中FBS和Hb A1c的水平均高于对照组,差异有统计学意义(t=4.28、6.06,P<0.05)。对4组不同HBV-DNA载量患者的FPG、Hb A1c的表达水平进行比较,差异有统计学意义(F=5.92、5.81,P<0.05),高HBV-DNA载量组患者FPG、Hb A1c表达水平均较其他组高。相关分析发现FPG、Hb A1c表达水平与HBV-DNA载量均呈正相关(r=0.463、0.491,P<0.05),随HBV-DNA载量增高,患者FBS、Hb A1c的表达水平也随之增高。结论慢性乙肝合并糖尿病患者血清HBV-DNA载量与FBS、Hb A1c升高可能存在一定相关性。
Objective To investigate the relationship between HBV-DNA load and fasting blood glucose (FPG) and glycated hemoglobin (HbA1c) in patients with chronic hepatitis B and with diabetes mellitus. Methods From November 2011 to October 2015, 115 CHB patients with diabetes mellitus admitted to Tangshan Infectious Disease Hospital of Hebei Province were enrolled in this study. Twenty-five healthy subjects at the same period were selected as the control group. Fasting blood glucose (FBS) and Hb Alc were measured in the early morning of the second day after admission, and serum HBV-DNA levels were detected by fluorescence quantitative PCR. Serum HBV-DNA load was used to divide the patients Divided into 4 groups, and analysis of HBV DNA content and FBS, Hb Alc correlation. Results The levels of serum FBS and Hb A1c in all patients were significantly higher than those in control group (t = 4.28,6.06, P <0.05). The levels of FPG and Hb A1c in four groups of patients with different HBV-DNA load were compared, the difference was statistically significant (F = 5.92,5.81, P <0.05). FPG, Hb A1c The expression levels were higher than other groups. Correlation analysis showed that the expression levels of FPG and Hb A1c were positively correlated with HBV-DNA load (r = 0.463,0.491, P <0.05). With the increase of HBV-DNA load, the expression of FBS and Hb A1c also increased . Conclusion There may be some correlation between the serum HBV-DNA load and the increase of FBS and Hb A1c in chronic hepatitis B patients with diabetes mellitus.
