目的 :探讨 型糖尿病患者无症状心肌缺血与微量白蛋白尿之间的关系 ,以利于冠心病的早期预防和治疗。方法 :将存在微量白蛋白尿的 型糖尿病患者 5 0例做为研究组 ,另选年龄、性别、糖尿病病程与之相匹配的无微量白蛋白尿症的 型糖尿病患者 32例做为对照组 ,采用次极量的踏车运动试验 ,记录心肌缺血的发生率、运动时间、最大运动量、高峰心率。结果 :研究组与对照组比较 ,心肌缺血反应的发生率较高 (6 5 %对 42 % ,P <0 .0 5 ) ,平均运动时间较短〔(6 .8± 2 .1)对 (7.9± 2 .0 ) min,P<0 .0 1〕,最大运动心率亦较低 (115± 2 0次 / min对 12 7± 16次 / min,P<0 .0 1) ,运动量较低 (6 41± 2 18kg· m / min对 740± 2 11kg·m/ min,P<0 .0 1)。结论 :存在微量白蛋白尿的 型糖尿病患者发生无症状心肌缺血较常见 ,通常在较低的运动负荷下就可发生心肌缺血 ,应对他们采取积极的冠心病预防措施
Objective: To investigate the relationship between asymptomatic myocardial ischemia and microalbuminuria in type 2 diabetic patients in order to facilitate the early prevention and treatment of coronary heart disease. Methods: Fifty patients with microalbuminuria who had microalbuminuria were selected as the study group. Thirty-two matched patients with microalbuminuria without age, gender and duration of diabetes were selected as the control group. Using the second most treadmill exercise test, record the incidence of myocardial ischemia, exercise time, the maximum amount of exercise, peak heart rate. Results: Compared with the control group, the incidence of myocardial ischemia was significantly higher in the study group than in the control group (65% vs 42%, P <0.05), and shorter mean exercise time was (6 .8 ± 2.1) (7.9 ± 2.0, min, P <0.01), and the maximum heart rate was also lower (115 ± 20 times / min vs 1277 ± 16 times / min, P <0.01) Low (6 41 ± 2 18 kg · m / min vs 740 ± 2 11 kg · m / min, P <0.01). Conclusions: Asymptomatic myocardial ischemia is common in patients with microalbuminuria, and myocardial ischemia usually occurs at lower exercise load, and should be followed up with positive measures of coronary heart disease prevention
