目的观察纤支镜微波治疗气管支气管肿瘤疗效。方法16例经纤支镜检查病理确诊的气管支气管肿瘤病人,胸片和胸部CT仅有肺不张或阻塞性肺炎,未发现肺内和纵隔占位病变,在纤支镜下行微波治疗并追踪。结果良性肿瘤8例,其中腺瘤5例,炎性肉芽肿3例,治疗2~6次,临床症状缓解,肺不张或阻塞性肺炎消失,纤支镜下见新生物均完全脱落,追踪0.5~9年未见复发。恶性肿瘤8例,其中鳞癌6例,腺癌1例,小细胞肺癌1例,微波治疗2~9次,临床症状减轻,3例肺不张复张,1例肺不张部分复张,纤支镜下见新生物均有缩小,3例完全脱落,追踪2个月~1年均复发或转移。结论微波治疗气管支气管肿瘤疗效确切,对良性肿瘤可治愈,替代手术治疗,对恶性肿瘤仅可作为一种辅助治疗方法。
Objective To observe the curative effect of bronchoscope microwave in the treatment of tracheobronchial tumors. Methods 16 patients with tracheobronchial tumors diagnosed by fibrobronchoscopy pathology had only atelectasis or obstructive pneumonia on chest X-ray and chest CT. No intrapulmonary and mediastinal lesions were found. Microwave therapy was followed by bronchoscopy. . Results 8 cases of benign tumors, including 5 cases of adenomas, 3 cases of inflammatory granulomas, 2 to 6 times of treatment, clinical symptoms, atelectasis or obstructive pneumonia disappeared, under the bronchoscope see new organisms are completely off, tracking 0.5 to 9 years no recurrence. 8 cases of malignant tumors, including 6 cases of squamous cell carcinoma, 1 case of adenocarcinoma, 1 case of small cell lung cancer, microwave treatment 2 to 9 times, clinical symptoms, 3 cases of atelectasis, 1 case of partial reexpansion of atelectasis, Under the bronchoscope, new organisms were seen to shrink, 3 cases completely detached, and they both relapsed or metastasized after 2 months to 1 year of follow-up. Conclusion Microwave therapy is effective for the treatment of tracheobronchial tumors. It is curable for benign tumors and can be used as an alternative treatment for malignant tumors.
