目的:探讨γ干扰素基因多态性与子宫颈HPV感染之间的相关性,为有效识别易感人群,有针对性地提出防治措施提供科学依据。方法:选取我院2009年5月~2012年12月收治的HPV感染阳性患者进入观察组,另选同期在我院住院的HPV感染阴性患者为对照组,采用等位基因特异引物-聚合酶链反应(ASP-PCR)技术,对γ干扰素基因rs2430561位点的基因型进行检测。结果:258例患者进入到本次研究,其中观察组85例,对照组173例。观察组AA基因型频率(41.18%,35/85)高于TT基因型(14.12%,12/85),A等位基因频率(64.71%,55/85)高于T等位基因频率(35.29%,30/85),差异有统计学意义(P<0.05)。调整年龄、病程和BMI因素后,Logistic回归结果显示,AA基因型是女性宫颈HPV感染的独立危险因素(OR=1.842,95%CI为1.102~3.924,P=0.028)。Kaplan Meier分析显示,AA基因型女性感染HPV的累积阴性率较AA、TA和TT基因型最小(1.03%~5.84%),差异有统计学意义(P<0.05)。对AA基因型研究对象进行为期3年的随访,宫颈HPV再次检出率为82.86%(29/35),TT基因型检出率为16.67%(2/12),差异有统计学意义(P<0.05)。结论:γ干扰素基因rs2430561位点的基因型对宫颈HPV感染的易感性有一定影响,AA基因型为宫颈HPV感染的独立危险因素。
Objective: To investigate the correlation between the polymorphism of γ interferon and cervical HPV infection, and to provide a scientific basis for proposing prevention and control measures for effectively identifying susceptible people. Methods: The patients with positive HPV infection admitted to our hospital from May 2009 to December 2012 were enrolled in the observation group. The patients with negative HPV infection who were hospitalized in our hospital during the same period were selected as the control group. Allele-specific primers - polymerase chain (ASP-PCR) technique was used to detect the rs2430561 genotype of IFN-γ gene. Results: 258 patients entered the study, including 85 cases in the observation group and 173 cases in the control group. The frequency of AA genotype (41.18%, 35/85) in observation group was higher than that in TT genotype (14.12%, 12/85), and the frequency of A allele (64.71%, 55/85) was higher than that of T allele %, 30/85), the difference was statistically significant (P <0.05). Logistic regression analysis showed that genotype AA was an independent risk factor for cervical HPV infection in women (OR = 1.842, 95% CI 1.102-3.924, P = 0.028) after adjusting for age, duration of disease and BMI. Kaplan Meier analysis showed that the cumulative negative rate of HPV infection in AA genotypes was the lowest (1.03% ~ 5.84%) compared with AA, TA and TT genotypes (P <0.05). The 3-year follow-up of AA genotypes showed that the detection rate of HPV was 82.86% (29/35) and that of TT genotype was 16.67% (2/12), the difference was statistically significant (P <0.05). CONCLUSION: The genotype of rs2430561 of interferon-γ gene has some influence on the susceptibility of cervical HPV infection. AA genotype is an independent risk factor of cervical HPV infection.
