目的探讨超声在子宫肌瘤和子宫腺肌病中的诊断价值。方法选择我院2009年8月-2011年8月经手术后病理组织学检查证实为子宫肌瘤患者40例,作为子宫肌瘤组。同时选择同期手术后病理组织学检查证实为子宫腺肌瘤40例,作为子宫腺肌瘤组。两组均行经腹和经阴道两种超声方法检查。记录经腹超声和经阴道超声两种方法下子宫肌瘤和子宫腺肌病诊断情况,并与手术后病理组织学检查结果比较,观察诊断符合率情况;观察子两组彩色多普勒超声下子宫动脉阻力、波动指数和血流量情况。结果子宫肌瘤组腹部超声诊断符合率与阴道超声比较,差异有统计学意义(P<0.05);子宫腺肌瘤组腹部超声诊断符合率与阴道超声比较,差异有统计学意义(P<0.05);子宫肌瘤组子宫动脉搏动指数、血流量显著高于子宫腺肌瘤组,差异有统计学意义(P<0.05);子宫肌瘤组子宫动血流阻力显著低于子宫腺肌瘤组,差异有统计学意义(P<0.05)。结论阴道超声对子宫肌瘤和子宫腺肌瘤都有较高的诊断价值,超声下子宫动脉阻力、子宫动脉搏动指数和血流量检测有助于鉴别子宫肌瘤和子宫腺肌瘤。
Objective To investigate the diagnostic value of ultrasound in uterine fibroids and adenomyosis. Methods Select our hospital from August 2009 to August 2011 by histopathological examination confirmed 40 cases of uterine fibroids, as the uterine fibroids group. At the same time select the same period after surgery histopathology confirmed adenomyosis in 40 cases, as the uterine adenomyosis group. Both groups were transabdominal and transvaginal ultrasonic examination. The diagnosis of uterine leiomyoma and adenomyosis under transabdominal sonography and transvaginal sonography were recorded and compared with the results of pathological examination after operation to observe the coincidence rate of diagnosis. The sub-groups were observed under color Doppler sonography Uterine artery resistance, fluctuation index and blood flow. Results The coincidence rate of abdominal ultrasound and uterine fibroids in uterine fibroids group was significantly higher than that of vaginal ultrasound (P <0.05). The coincidence rate of abdominal ultrasound and vaginal ultrasound in uterine adenoma group was statistically significant (P <0.05 ); Uterine fibroids uterine artery pulse index, blood flow was significantly higher than the uterine adenomyosis group, the difference was statistically significant (P <0.05); uterine fibroids uterine blood flow resistance was significantly lower than the uterine adenomyoma , The difference was statistically significant (P <0.05). Conclusions Vaginal ultrasound has high diagnostic value for uterine fibroids and adenomyoma. Uterine arterial resistance, uterine artery pulse index and blood flow measurement can help to distinguish uterine fibroids and adenomyomas.
