目的:研究西宁地区健康孕产妇的甲状腺激素参考值范围及变化规律。方法:采用Immulite2000电化学发光仪测定本地区单胎妊娠健康妇女早、中、晚孕期及年龄匹配非妊娠妇女的甲状腺激素水平并建立各自的参考范围。结果:早、中、晚孕妇女及非妊娠妇女的甲状腺激素水平均呈偏态分布,以中位数(M)及双侧限值(P2.5~P97.5)表示其参考范围。血清促甲状腺激素(TSH)在早[2.20(2.08~2.36)]、中[2.74(2.64~3.84)]、晚孕期[3.01(2.69~3.33)]逐渐上升,各期两两比较差异有统计学意义(P<0.05),晚孕期略低于非妊娠妇女[3.11(1.02~4.04)],但差异无统计学意义(P>0.05)。血清游离三碘甲腺原氨酸(FT3)在妊娠的早期[3.91(3.85~3.97)];中期[3.74(3.63~3.84)]、晚期[3.35(3.13~3.56)];血清游离甲状腺素(FT4)在早期[1.19(1.08~1.20)];中期[1.00(0.99~1.31)]、晚孕期[0.87(0.83~0.95)]逐渐降低,各期两两比较差异有统计学意义(P<0.05),早孕期低于非妊娠妇女FT3[4.12(4.04~6.13)]、FT4[1.34(1.17~1.69)],且差异有统计学意义(P<0.05)。结论:西宁地区正常孕妇的甲状腺激素水平明显不同非妊娠妇女,早、中、晚孕期之间也存在显著差异[1],建立西宁地区正常孕妇妊娠期甲状腺激素参考范围将有助于妊娠期甲状腺功能紊乱的临床诊疗。
Objective: To study the range and change of thyroid hormone reference value of healthy pregnant women in Xining area. Methods: The thyroid hormone levels in early, middle and late pregnancy and age-matched nonpregnant women in single-pregnancy women were determined by Immulite2000 electrochemiluminescence apparatus and their respective reference ranges were established. Results: The levels of thyroid hormones in preterm, middle and late pregnant women and nonpregnant women showed a skewed distribution, with the median (M) and bilateral limit (P2.5 ~ P97.5) as the reference range. Serum thyroid stimulating hormone (TSH) gradually increased in early [2.20 (2.08 ~ 2.36)], [2.74 (2.64 ~ 3.84)] and late pregnancy [3.01 (2.69 ~ 3.33) (P <0.05), but slightly lower than that of non-pregnant women [3.11 (1.02-4.04)] in late pregnancy, but the difference was not statistically significant (P> 0.05). Serum free triiodothyronine (FT3) was significantly higher in early pregnancy [3.91 (3.85-3.97)], in interim [3.74 (3.63-3.84) FT4 decreased gradually in early stage [1.19 (1.08-1.20)], mid stage (1.00 (0.99-1.31)] and late pregnancy (0.87-0.35-0.95), with significant difference in each period (P <0.05) ) In early pregnancy was lower than that in nonpregnant women (4.12 (4.04 ~ 6.13)] and FT4 [1.34 (1.17 ~ 1.69)], and the difference was statistically significant (P <0.05).
