[目的]从有骨力学负荷意义的三个常用体测指标体重、身高和体重指数(BMI)中找出与峰值骨量(PBM)相关最密切的指标,并进行PBM的标准化以用于OP的诊断。[方法]纳入114例20~40岁青年男女样本,比较不同部位的骨量(BMC、BMD)与体重、身高和BMI的相关关系。在此基础上选出相关系数最佳,标准化其骨量,并计算其变异系数是否适合用于骨质疏松的诊断。[结果]①不论男女,BMC与体重、身高和BMI3个指标的相关都强于BMD与这3个指标的相关,并且BMC与体重的相关强于BMC与身高和BMI的相关,其中女性的下肢、躯干和全身BMC与体重的相关最为密切(r=0.8466、0.8770和0.8470)。②经体重标准化(BMD/BMC÷体重)后男女间各部位的BMC较BMD更接近相同。③全身各部位BMDBMC中,下肢、躯干和全身的BMC经体重标化后的变异系数最小(11.48%~13.54%),均接近10%。[结论]使用体重标准化BMC的指标可用于OP诊断,能较好排除骨力学负荷对骨量的影响。可首先应用与女性的诊断,应用部位以下肢、躯干和全身为佳。
[Objective] To find out the most relevant index of peak bone mass (PBM) from the three common physical measurement indexes of body weight, height and body mass index (BMI) in the sense of mechanical load and to standardize PBM for OP diagnosis. [Methods] A total of 114 young men and women aged from 20 to 40 were enrolled in this study. The bone mass (BMC, BMD) at different sites were compared with body weight, height and BMI. On this basis, the best correlation coefficient is selected, the amount of bone mass is normalized, and whether the coefficient of variation is suitable for the diagnosis of osteoporosis is calculated. [Results] ①Both of men and women, the correlation between BMC and body mass, height and BMI were stronger than that between BMD and these three indexes, and the correlation between BMC and body weight was stronger than that between BMC and height and BMI. Body mass and body weight were most closely related to trunk and systemic BMCs (r = 0.8466, 0.8770 and 0.8470). ② BMCs in different parts of men and women after BMD / BMC ÷ body weight are more similar than BMD. (3) The coefficient of variation (BMI) of BMD BMC in the lower extremity, body trunk and whole body of BMD ’s BMC in the whole body was the smallest (11.48% ~ 13.54%), which were close to 10%. [Conclusion] The indexes of using BMC standardized by body weight can be used for diagnosis of OP, which can better exclude the influence of bone load on bone mass. May be the first application and diagnosis of women, the application site to the lower limbs, trunk and body is better.
