目的:制作压榨性损伤颞骨内面瘫实验模型,探讨颞骨内面神经麻痹面神经减压术的意义。方法:测定面神经骨管开放组和非开放组面神经膨胀率,并行光镜、电镜观察。应用肌电图仪测试神经兴奋传导恢复时间和瞬目反射恢复时间。结果:骨管开放组面神经膨胀率为(45.7±7.0)%,非开放组为(31.1±5.2)%,两组间差异十分显著(P<0.01)。神经兴奋传导恢复时间,开放组为(35.6±7.0)d,非开放组(74.1±10.6)d,两组间差异也十分显著(P<0.01)。非开放组比开放组神经纤维间的浮肿程度轻,而髓鞘板层间开离现象重。结论:开放面神经管能解除骨管对面神经的反作用力损伤,面神经减压术有利于面神经功能恢复
Objective: To make an experimental model of intractable facial palsy with crush injury and explore the significance of decompression of facial nerve with paralyzed facial nerve in the temporal bone. Methods: The facial nerve expansion rate of open facial nerve group and non-open group was measured by light and electron microscope. Electromyography was used to test the recovery time of nerve excitement conduction and instantaneous reflex recovery time. Results: The rate of facial nerve expansion was (45.7 ± 7.0)% in open group and (31.1 ± 5.2)% in open group. The difference between the two groups was significant (P <0.01). The recovery time of nerve conduction was 35.6 ± 7.0 days in open group and 74.1 ± 10.6 days in non-open group. The difference between the two groups was also significant (P <0.01). The degree of edema in the non-open group was lighter than that in the open group, and the delamination of the myelin sheath was heavy. Conclusion: Open facial nerve canal can relieve the reaction of the facial nerve to the facial nerve, and facial nerve decompression is good for facial nerve function recovery
