目的探讨尿微量白蛋白(MAU)和β2-微球蛋白(β2-MG)在新生儿支原体肺炎中的表达及意义。方法选择2015年9月-2016年9月在秦皇岛市第二医院进行治疗的肺炎新生儿200例为研究对象。其中97例为支原体肺炎患儿(观察组),另103例为其他病原体肺炎患儿(对照组),对比两组MAU、β2-MG、肌酐(Scr)及血尿素氮(BUN)水平及阳性率,比较不同肺部感染评分(CPIS)对应的MAU及β2-MG水平,分析观察组CPIS评分与MAU及β2-MG的相关性。结果观察组的MAU及β2-MG水平明显高于对照组,差异均有统计学意义(P<0.05)。两组的Scr及BUN水平比较,差异无统计学意义(P>0.05)。观察组的MAU阳性率为57.73%,β2-MG阳性率为43.30%,明显高于对照组的2.91%及1.94%,差异均有统计学意义(P<0.05)。观察组CPIS评分为5~8分及9~12分的MAU及β2-MG水平明显高于≤4分的水平,差异有统计学意义(P<0.05)。观察组CPIS评分为9~12分的MAU及β2-MG水平明显高于5~8分的水平,差异有统计学意义(P<0.05)。Pearson相关性发现,观察组CPIS评分与MAU及β2-MG均呈正相关(r=0.763,0.694;P=0.000,0.000)。结论 MAU及β2-MG在支原体肺炎新生儿中水平较高,能有效反应感染的程度,可一定程度地预测肾功能损伤。
Objective To investigate the expression and significance of urinary microalbumin (MAU) and β2-microglobulin (β2-MG) in neonatal Mycoplasma pneumonia. Methods A total of 200 neonates with pneumonia who were treated in the Second Hospital of Qinhuangdao from September 2015 to September 2016 were selected as the study subjects. 97 cases were children with mycoplasma pneumonia (observation group) and 103 cases were children with other pathogen pneumonia (control group). The MAU, β2-MG, creatinine (Scr) and blood urea nitrogen (BUN) Rates of MAU and β2-MG corresponding to different lung infection scores (CPIS) were compared. The correlation between CPIS score and MAU and β2-MG in the observation group was analyzed. Results The levels of MAU and β2-MG in the observation group were significantly higher than those in the control group, with significant differences (P <0.05). There was no significant difference in Scr and BUN between the two groups (P> 0.05). The positive rate of MAU in the observation group was 57.73%, the positive rate of β2-MG was 43.30%, which was significantly higher than that of the control group (2.91% and 1.94%, P <0.05). The MAU and β2-MG levels in the observation group with CPIS score of 5 to 8 and 9 to 12 were significantly higher than those of ≤4, the difference was statistically significant (P <0.05). The level of MAU and β2-MG in observation group with CPIS score of 9-12 was significantly higher than that of 5-8, the difference was statistically significant (P <0.05). Pearson correlation found that the CPIS score of the observation group was positively correlated with MAU and β2-MG (r = 0.763,0.694; P = 0.000,0.000). Conclusions The level of MAU and β2-MG in neonates with mycoplasma pneumonia is higher, which can effectively reflect the degree of infection and can predict renal function injury to a certain extent.
