目的探讨中性粒细胞与淋巴细胞比值(NLR)与腹膜透析患者动脉硬化的关系。方法收集2014年11月至2016年9月我院肾内科进行腹膜透析治疗的140例患者,运用根据肱-踝脉搏传导速度(ba PWV)评估其动脉硬化程度,并根据ba PWV值分为低ba PWV组和高ba PWV组。收集生理生化资料,分析NLR和ba PWV的关系。结果高ba PWV组NLR高于低ba PWV组(P<0.05)。年龄、吸烟、糖尿病、心血管病史、MAP、净超滤量、hs CRP、WBC、中性粒细胞及NLR与ba PWV呈正相关;白蛋白、磷水平、甲状旁腺激素水平、淋巴细胞计数与ba PWV呈负相关(P<0.05)。线性回归分析发现NLR是ba PWV的独立预测因子,NLR每变化一个单位,ba PWV增加35.5%。结论 NLR是腹膜透析患者动脉硬化的独立预测因子,仍需前瞻性的队列研究其因果关系。
Objective To investigate the relationship between neutrophil and lymphocyte ratio (NLR) and atherosclerosis in peritoneal dialysis patients. Methods A total of 140 patients undergoing peritoneal dialysis in our Department of Nephrology from November 2014 to September 2016 were enrolled in this study. The degree of atherosclerosis was assessed according to the brachial-ankle pulse rate (ba PWV) and classified according to the value of ba PWV ba PWV group and high ba PWV group. Physiological and biochemical data were collected to analyze the relationship between NLR and ba PWV. Results The NLR of high ba PWV group was higher than that of low ba PWV group (P <0.05). The age, smoking, diabetes, history of cardiovascular disease, MAP, net ultrafiltration volume, hs CRP, WBC, neutrophil and NLR were positively correlated with ba PWV; albumin, phosphorus levels, parathyroid hormone levels, lymphocyte counts and ba PWV was negatively correlated (P <0.05). Linear regression analysis showed that NLR was an independent predictor of ba PWV, with a PWU of 35.5% increased for each unit of change in NLR. Conclusions NLR is an independent predictor of atherosclerosis in patients undergoing peritoneal dialysis and a prospective cohort study of causality is still needed.
