目的总结奎屯地区食管癌的治疗经验。方法回顾性分析600例食管癌患者采用不同治疗手段的相关临床资料。结果食管癌患者600例,其中,上段184例,中段220例,下段196例;通过影像学检查有区域淋巴结转移的患者378例,有远处转移患者151例,未发现淋巴结转移的患者222例;单纯手术组104例,单纯放射治疗组92例,单纯化学治疗组68例,手术后放射治疗组97例,手术后化学治疗组84例,放射治疗加化学治疗组80例,营养支持组75例;随访6个月至3年,1年生存率、3年生存率分别为:单纯手术组80.7%、46.2%;单纯放射治疗组52.2%、28.3%;单纯化学治疗组29.4%、8.8%;手术后放射治疗组79.4%、42.3%;手术后化学治疗组72.6%、29.8%;放射治疗加化学治疗组57.5%、21.3%;营养支持组12.0%、2.6%。结论无淋巴结转移的食管癌患者手术、单纯放射治疗可以取得相同的疗效,但后者生活质量明显提高,有区域淋巴结转移的食管癌患者手术后行辅助放射治疗可以明显提高生存率。
Objective To summarize the treatment experience of esophageal cancer in Kuitun area. Methods Retrospective analysis of 600 cases of esophageal cancer patients using different methods of treatment of clinical data. Results Among the 600 patients with esophageal cancer, there were 184 cases in the upper part, 220 cases in the middle part and 196 cases in the lower part. There were 378 cases with regional lymph node metastasis by imaging examination, 151 cases with distant metastasis, 222 cases without lymph node metastasis ; Simple surgery group 104 cases, simple radiotherapy group 92 cases, simple chemotherapy group 68 cases, postoperative radiotherapy group 97 cases, postoperative chemotherapy group 84 cases, radiotherapy plus chemotherapy group 80 cases, nutritional support group 75 The follow-up ranged from 6 months to 3 years. The 1-year survival rate and 3-year survival rate were 80.7% and 46.2% in the simple operation group, 52.2% and 28.3% in the radiotherapy alone group, 29.4% and 8.8% in the simple chemotherapy group, ; The postoperative radiotherapy group was 79.4% and 42.3% respectively; the postoperative chemotherapy group was 72.6% and 29.8%; the radiotherapy and chemotherapy group was 57.5% and 21.3%; the nutrition support group was 12.0% and 2.6% respectively. Conclusions Surgery and radiotherapy alone without lymph node metastasis can achieve the same effect, but the quality of life of the latter patients is obviously improved. Adjuvant radiotherapy after esophageal cancer patients with regional lymph node metastasis can significantly improve the survival rate.
