目的:调查甘肃省宫颈癌高发区妇女子宫颈上皮内人乳头状瘤病毒(human papillomavirus,HPV)感染状况及其危险因素。方法:采用横断面研究,于2005年在甘肃省陇南市武都区以整群抽样的方法选择30~49岁、有性生活史的农村女性人群进行HPV感染状况调查、宫颈脱落细胞HPV DNA检测及细胞学和病理学诊断。所有女性按年龄分层入组,进行问卷调查、宫颈脱落细胞采集、第二代杂交捕获试验检测HPV。结果:2 052例受检者中,HPV总感染率为7.89%(168/2 052),其中,宫颈正常人群HPV感染率为5.49%(108/1 968),CINⅠ妇女HPV感染率为47.06%(16/34),CINⅡ及以上病变的妇女HPV感染率为78.13%(25/32)。随着宫颈上皮内瘤样病变程度增加,HPV感染率明显上升(χ2=17.996,P<0.001)。HPV感染与丈夫婚外性行为、宫颈柱状上皮异位、尿路感染、阴道滴虫感染等因素有关。结论:武都区30~49岁妇女中HPV感染率为7.89%;随着宫颈上皮内瘤样病变程度的增加,HPV感染率增加;影响HPV感染率的危险因素有丈夫婚外性行为、妇女宫颈柱状上皮异位、尿路感染、阴道滴虫感染等。
Objective: To investigate the status of cervical intraepithelial human papillomavirus (HPV) infection and its risk factors in women with high incidence of cervical cancer in Gansu Province. Methods: A cross-sectional study was conducted in 2005 to investigate the status of HPV infection in rural women aged 30-49 years with sexual life history by cluster sampling method in Wudu District, Longnan City, Gansu Province. HPV DNA level in cervical exfoliated cells And cytology and pathology diagnosis. All women stratified by age, conducted a questionnaire survey, cervical exfoliative cells collected, the second generation hybridization capture test to detect HPV. Results: Among 2 052 subjects, the total HPV infection rate was 7.89% (168/2 052). Among them, HPV infection rate was 5.49% (108/1 968) in normal cervix and 47.06% in CIN Ⅰ women (16/34). The HPV infection rate of women with CINⅡ and above lesions was 78.13% (25/32). As the degree of cervical intraepithelial neoplasia increased, the HPV infection rate increased significantly (χ2 = 17.996, P <0.001). HPV infection and her husband extramarital sex, cervical columnar epithelial ectopic, urinary tract infection, vaginal trichomoniasis and other factors. Conclusion: The HPV infection rate in women aged 30-49 years in Wudu District was 7.89%. HPV infection rate increased with the degree of cervical intraepithelial neoplasia. The risk factors of HPV infection rate were husband’s extramarital sex, Cervical columnar epithelium ectopic, urinary tract infection, vaginal trichomoniasis infection.
