目的分析比较某社区中年人群体质指数(body mass index,BMI)和腰围身高比(waist-to-height ratio,WHtR)与血压的关系及其对高血压的预测效果。方法以在金昌市金川区某社区开展的健康行为因素调查中4 088例中年人为研究对象,采用线性回归模型比较血压(收缩压和舒张压)随BMI及WHtR的数量变化,并用受试者工作特征(receiver operating characteristic,ROC)曲线评价BMI及WHtR对高血压的预测效果及适宜切点。结果该社区中年人群的收缩压和舒张压均随着BMI、WHtR的增大而升高,且BMI对血压的升高作用大于WHtR。多元线性回归分析中,WHtR与不同性别中年人群收缩压、舒张压均无相关关系。男性BMI较WHtR对高血压的预测效果更优(Z=2.06,P=0.020),女性BMI较WHtR对高血压的预测效果也更优(Z=1.77,P=0.038)。结论该社区中年人群中BMI与血压(收缩压和舒张压)的相关性较WHtR强,应将BMI≥24.0 kg/m2和(或)WHtR≥0.5的中年人群做为该社区高血压防控的重点人群。
Objective To analyze and compare the relationship between body mass index (BMI) and waist-to-height ratio (WHtR) and blood pressure in a community and to predict its effect on hypertension. Methods A total of 4 088 middle-aged adults in a community survey conducted in Jinchuan District of Jinchang City were used to compare the changes of blood pressure (systolic and diastolic blood pressure) with BMI and WHtR using a linear regression model. The receiver operating characteristic (ROC) curve was used to evaluate the predictive value of BMI and WHtR for hypertension and the suitable cut point. Results The systolic and diastolic blood pressure of middle-aged population in this community increased with the increase of BMI and WHtR, and the effect of BMI on the increase of blood pressure was greater than that of WHtR. In multivariate linear regression analysis, there was no correlation between WHtR and systolic blood pressure and diastolic blood pressure in middle-aged people of different genders. Compared with WHtR, male BMI had better prediction of hypertension (Z = 2.06, P = 0.020), and female BMI had better prediction of hypertension than WHtR (Z = 1.77, P = 0.038). Conclusion The correlation between BMI and blood pressure (systolic and diastolic blood pressure) in middle-aged population of this community is stronger than that of WHtR. Middle-aged population with BMI≥24.0 kg / m 2 and / or WHtR≥0.5 should be regarded as hypertension prevention Control key people.
