目的探讨2型糖尿病(T2DM)患者左、右颈动脉内膜中层厚度(IMT)与心血管危险因素的关系。方法根据糖化血红蛋白1c(HbA1c)水平将272例T2DM患者分为控制满意组(HbA1c≤8%,A组,94例)和控制不满意组(HbA1c>8%,B组,178例),用超声多普勒检查双侧颈动脉内膜中层厚度,并测定血糖和血脂水平。结果 B组左侧IMT(L-IMT)和右侧IMT(R-IMT)均较A组增厚(P<0.05),空腹血糖、总胆固醇和低密度脂蛋白胆固醇也较A组增高(P<0.05)。A组L-IMT、R-IMT与年龄、收缩压、脉压呈正相关(P<0.05),与高密度脂蛋白胆固醇呈负相关(P<0.05)。B组L-IMT、R-IMT与年龄、脉压呈正相关(P<0.05)。结论良好的血糖控制,能够降低T2DM患者心血管疾病发生风险。
Objective To investigate the relationship between left and right carotid intima-media thickness (IMT) and cardiovascular risk factors in type 2 diabetes mellitus (T2DM). Methods According to the level of HbA1c, 272 patients with T2DM were divided into control satisfaction group (HbA1c≤8%, A group, 94 cases) and control dissatisfaction group (HbA1c> 8%, B group, 178 cases) Doppler ultrasound examination of bilateral carotid artery intima-media thickness, and determination of blood glucose and blood lipid levels. Results The left IMT (IMT) and the right IMT (IMT) in group B were thicker than those in group A (P <0.05), and the fasting blood glucose, total cholesterol and LDL cholesterol were also increased <0.05). L-IMT and R-IMT in group A were positively correlated with age, systolic blood pressure and pulse pressure (P <0.05), and negatively correlated with high density lipoprotein cholesterol (P <0.05). B-group L-IMT, R-IMT and age, pulse pressure was positively correlated (P <0.05). Conclusion Good glycemic control can reduce the risk of cardiovascular disease in T2DM patients.
