目的探讨轮状病毒(RV)肠炎患儿血清免疫球蛋白(Ig)和补体(C)的变化及其在发病机制中的作用。方法对2004年7月至2005年9月广东省东莞市人民医院收治的81例RV肠炎患儿及22例健康儿童(对照组),利用透射比浊法进行血清IgG、IgA、IgM和补体C3、C4质量浓度的测定。结果RV肠炎患儿急性期血清IgG、IgA、IgM和C3质量浓度较恢复期及正常对照组明显降低(P<0·01或<0·05)。RV肠炎中、重型患儿血清IgG、IgA、IgM和C3明显低于轻型患儿(P<0·01或<0·05)。IgG、IgA、IgM和C3质量浓度与病情严重程度呈负相关(r分别为-0·389,-0·547,-0·775,-0·743,均P<0·01)。C4质量浓度与病情无相关性(rs=-0·061,P>0·05)。结论RV肠炎患儿体液免疫活性变化在RV肠炎发病机制中起重要作用,对判断病情严重程度和指导治疗有一定实用价值。
Objective To investigate the changes of serum immunoglobulin (Ig) and complement (C) in children with rotavirus (RV) enteritis and its role in pathogenesis. Methods Eighty-one children with RV enteritis and 22 healthy children (control group) admitted to Dongguan People’s Hospital of Guangdong Province from July 2004 to September 2005 were enrolled in this study. Serum IgG, IgA, IgM and complement C3 , C4 mass concentration determination. Results Serum levels of IgG, IgA, IgM and C3 in children with RV enteritis were significantly lower than those in the recovery and normal controls (P <0.01 or 0.05). In RV enteritis, serum IgG, IgA, IgM and C3 were significantly lower in severe children than in mild children (P <0.01 or <0.05). The concentrations of IgG, IgA, IgM and C3 were negatively correlated with the severity of the disease (r = -0.389, -0.547, -0.775, -0.743, all P <0.01). There was no correlation between C4 concentration and disease (rs = -0.061, P> 0.05). Conclusion The changes of humoral immunity in children with RV enteritis play an important role in the pathogenesis of RV enteritis and have some practical value in judging the severity of the disease and guiding the treatment.
