目的:通过对人Barrett食管(BE)、食管腺癌(EA)、进行MUC1、MUC6的检测,揭示上述2种因子在BE、EA发生发展中的作用及临床意义。方法:经胃镜、病理确诊的BE患者50例,取其粘膜作为BE组,取胃镜活检或手术切除经病理确诊的EA标本25例,作为EA组。分别取正常食管鳞状上皮20例作为正常对照组。进行免疫组化染色观察各组标本中MUC1、MUC6蛋白的表达情况,并进行组间比较。结果:①BE组MUC1的表达高于正常食管鳞状上皮组(P<0.01),而低于EA组(P<0.01);BE组MUC6表达高于正常食管鳞状上皮组(P<0.01),且高于EA组(P<0.01)。②MUC1蛋白的表达与EA细胞分化程度呈负相关(P<0.05),与EA病理学分期呈正相关(P<0.05);MUC6蛋白的表达与EA的临床病理因素无关(P>0.05)。结论:MUC1、MUC6的表达变化可能与BE、EA的发生发展密切相关,MUC1、MUC6与EA的某些临床病理因素相关,监测这些指标可能为EA的诊疗预后提供参考价值。
OBJECTIVE: To detect the expression of MUC1 and MUC6 in human Barrett’s esophagus (BE) and esophageal adenocarcinoma (EA) and to reveal the role and clinical significance of these two factors in the development of BE and EA. Methods: Fifty patients with endometrial cancer diagnosed by endoscopy and pathology were enrolled in this study. Twenty-five patients with pathologically diagnosed EA were examined by endoscopy or biopsy with the mucosa of the BE group. Twenty normal esophageal squamous cells were taken as normal control group respectively. Immunohistochemical staining was used to observe the expression of MUC1 and MUC6 protein in each group and compare between groups. Results: ① The expression of MUC1 in BE group was significantly higher than that in normal esophageal squamous epithelium group (P <0.01), but lower than that in EA group (P <0.01); MUC6 expression in BE group was higher than that in normal esophageal squamous epithelium group (P <0.01) And higher than EA group (P <0.01). ② The expression of MUC1 protein was negatively correlated with the differentiation degree of EA cells (P <0.05), and positively correlated with the pathological stage of EA (P <0.05). The expression of MUC6 protein had no correlation with the clinicopathological parameters of EA (P> 0.05). CONCLUSION: The changes of MUC1 and MUC6 expression may be closely related to the occurrence and development of BE and EA. MUC1 and MUC6 are associated with some clinicopathological factors of EA. Monitoring these indexes may provide reference value for diagnosis and treatment of EA.
