目的探讨妊娠期糖尿病(GDM)孕妇孕期采用食品交换份的营养干预管理对体重指数、糖代谢指标和血脂水平指标控制及妊娠结局的作用。方法以2013年1月—2014年9月在宁波市江北区妇幼保健院进行常规产检的348例妊娠期糖尿病孕妇为研究对象,自愿接受个体化食品交换份干预指导的226例为研究组,未接受系统指导营养干预的122例为对照组,比较两组孕妇的体重指数、糖代谢指标和血脂水平指标、妊娠并发症和妊娠结局等情况。结果研究组孕妇经过个体化营养指导及食品交换份干预后,平均体重指数增长明显低于对照组,差异有统计学意义(P<0.05),糖代谢指标和血脂水平指标控制良好,与对照组比较差异有统计学意义(P<0.05),妊娠并发症及不良妊娠结局的发生率均明显低于对照组,差异有统计学意义(P<0.05)。结论食品交换份法的营养干预管理可有效地控制血糖与改善妊娠结局,保障母婴健康,值得临床推广应用。
Objective To investigate the effect of nutritional interventions for pregnant women with gestational diabetes mellitus (GDM) on the control of body mass index, glycometabolism and blood lipid levels and pregnancy outcomes during pregnancy. Methods A total of 348 pregnant women with gestational diabetes mellitus who were routinely diagnosed at Maternal and Child Health Hospital of Jiangbei District in Ningbo City from January 2013 to September 2014 were selected as research objects and 226 volunteers who were given the guidance of exchanging individualized food for intervention were selected as the research group. The 122 patients who underwent systematic guidance of nutritional intervention served as the control group. Body mass index, glucose metabolism index, blood lipid level, pregnancy complications and pregnancy outcome were compared between the two groups. Results After the individualized nutrition guidance and the food exchange intervention, the average body mass index of the pregnant women in the research group increased significantly lower than that in the control group (P <0.05), the indicators of glucose metabolism and blood lipid level were well controlled. Compared with the control group The difference was statistically significant (P <0.05), the incidence of pregnancy complications and adverse pregnancy outcomes were significantly lower than the control group, the difference was statistically significant (P <0.05). Conclusion The nutritional intervention management of the food exchange method can effectively control the blood sugar and improve the pregnancy outcome, and ensure the health of the mother and the infant. It is worthy of clinical application.
