目的分析影响尿沉渣分析仪检测小圆上皮细胞的影响因素。建立唐山市区正常人群小圆上皮细胞的参考区间。方法收集2015年11月-2016年2月住院和门诊患者尿沉渣标本2 403份,使用UF-1000i尿沉渣分析仪分析和显微镜镜检,对数据进行统计得出尿分析仪的准确率,分析导致小圆上皮细胞检测错误的影响因素;随机收集体检健康人群尿液标本1 200份,确立仪器法95%尿沉渣小圆上皮细胞参考区间。结果 UF-1000i尿流式细胞仪检测小圆上皮细胞的假阳性率为18.35%,假阴性率为0.84%。导致假阳性的主要影响因素为尿中的白细胞、形态较小的表面上皮细胞和滴虫等。对含有以上3种细胞的尿液标本中的小圆上皮细胞数据进行统计,表明分析仪和镜检结果的差异均有统计学意义(t值分别为6.41、3.23、5.43,P<0.05)。唐山市区尿沉渣小圆上皮细胞的参考区间:男性为0个/μl~3.40个/μl:女性为0个/μl~3.64个/μl。结论 UF-1000i的准确性较高,其中假阴性率极低,但假阳性率较高,导致假阳性的主要因素为白细胞等细胞增高引起的分析仪误判,因此临床检测时遇到白细胞等细胞增高的标本要进行显微镜镜检。
Objective To analyze the influencing factors of urinary sediment analyzer in detecting small round epithelial cells. Establishment of Tangshan urban normal small round epithelial cells reference interval. Methods A total of 2 403 urinary sediment samples from hospitalized patients and outpatients from November 2015 to February 2016 were collected and analyzed by UF-1000i urine sediment analyzer and microscopic examination. The data were analyzed to get the accuracy of the urinalysis analyzer. Resulting in small round of epithelial cells detected in the wrong factors; random collection of urine samples from healthy volunteers 1,200 copies, the establishment of 95% urinary sediment small round epithelial cells reference interval. Results UF-1000i uroflow cytometry detection of small round epithelial cells, the false positive rate was 18.35%, the false negative rate was 0.84%. The main factors that lead to false positives are leukocytes in urine, small surface epithelial cells and trichomoniasis. Statistics of the small round epithelial cells in the urine samples containing the above three kinds of cells showed that the differences between the analyzer and the microscopic examination were statistically significant (t = 6.41,3.23,5.43, P <0.05). Urinary sediment in Tangshan small circular epithelial cells reference interval: 0 male / μl ~ 3.40 / μl: 0 female / μl ~ 3.64 / μl. Conclusion The accuracy of UF-1000i is high. The false negative rate is very low, but the false positive rate is high. The main factor leading to false positives is the erroneous analyzer caused by the increase of leukocytes and other cells. Cells with elevated specimens should be microscopically examined.
