目的通过37万余例子宫颈液基细胞学筛查结果分析,探讨广东地区子宫颈癌及癌前病变的发病情况。方法2002年10月~2006年12月,对广东地区371929例妇女采用液基薄层细胞学(TCT)方法筛查子宫颈病变。细胞学诊断按TBS(the Bethesda system)标准(2001版改良TBS诊断标准),按年龄及地区分别统计细胞学阳性率。结果广东地区371929例妇科临床子宫颈标本,细胞学阴性331251例,占91.23%;细胞学阳性病例:鳞状上皮病变32548例(8.96%),腺上皮病变486例(0.13%);年龄分组:30~40岁阳性率为9.13%,40~50岁阳性率为9.60%,后者细胞学阳性率较高;地区分组:珠三角地区标本总数304951例,占81.99%,细胞学筛查阳性者24301例(8.14%);非珠三角地区66978例(18.01%),细胞学筛查阳性者7645例(11.87%),2004~2005年有组织病理活检/随访结果的共1858例,随访/活检率为2.88%。结论广东地区子宫颈癌及癌前病变的细胞学阳性率为9.09%(其中细胞学诊断癌前病变率为3.40%),30~50岁年龄组是癌前病变及癌的高发期,亦是子宫颈癌预防的主要阶段。经济不发达的非珠三角地区细胞学阳性率为11.87%,远高于珠三角地区。
Objective To investigate the incidence of cervical cancer and precancerous lesions in Guangdong Province through the screening of more than 370,000 cases of cervical liquid-based cytology. Methods From October 2002 to December 2006, 371929 women in Guangdong Province were screened for cervical lesions by liquid-based thin-layer cytology (TCT). Cytological diagnosis by TBS (the Bethesda system) standard (2001 version of improved TBS diagnostic criteria), by age and region, respectively, cytology-positive rate statistics. Results There were 371,991 cases of gynecological clinical cervical specimens in Guangdong area, with 331251 cases negative, accounting for 91.23%. The positive cases of cytology were 32548 cases (8.96%) with squamous epithelial lesions and 486 cases (0.13% The positive rate of 30 ~ 40 years old was 9.13%, the positive rate of 40 ~ 50 years old was 9.60%, while the latter was the high positive rate of cytology. Regional grouping: The total number of specimens in the Pearl River Delta was 304,951, accounting for 81.99% 24,301 cases (8.14%), 66,978 cases (18.01%) in non-Pearl River Delta region, 7,645 cases (11.87%) of cytology screening positive cases, 1858 cases of biopsy / follow-up results from 2004 to 2005, The rate is 2.88%. Conclusions The positive rate of cervical cytology and precancerous lesions in Guangdong province is 9.09% (among them, the rate of precancerous lesion in cytology is 3.40%). The incidence of precancerous lesions and cancers in 30 ~ 50 age group is also high The main stage of cervical cancer prevention. The undeveloped non-PRD cytology positive rate was 11.87%, far higher than the Pearl River Delta region.
