目的：探讨嗅神经母细胞瘤的诊断和治疗效果。方法：２２例患者，１５例常规病理检查明确诊断，７例用ＮＳＥ、ＮＦ、ＬＣＡ、ＥＭＡ、ＣＥＡ抗体行免疫组化检查。所有病例分别采用手术、放疗、手术加放疗、手术加放疗和化疗进行治疗。结果：ＮＳＥ阳性率７／７，ＮＦ５／７，其余３种抗体检查均为阴性。１９例手术中１４例发现前颅窝底骨质破坏。随访１７例，３年、５年生存率分别为５９％和３５％。结论：免疫组化检测对本病诊断和鉴别诊断有较大帮助，病变位于鼻腔顶部且前颅窝底骨质破坏提示本病的可能。早期诊断加综合治疗能提高本病生存率。
Objective: To investigate the diagnosis and treatment of olfactory neuroblastoma. Methods: Twenty-two patients were diagnosed by routine pathological examination and immunohistochemistry was performed in 7 patients with NSE, NF, LCA, EMA and CEA antibodies. All cases were treated with surgery, radiotherapy, surgery plus radiotherapy, surgery plus radiotherapy and chemotherapy. Results: The positive rate of NSE was 7/7, NF5 / 7, the other three antibodies were negative. In 19 cases, 14 cases found the destruction of anterior skull base bone. Follow-up 17 cases, 3-year, 5-year survival rates were 59% and 35%. Conclusion: Immunohistochemical detection of the disease diagnosis and differential diagnosis of a greater help, the lesion is located in the top of the nasal cavity and bony destruction of the anterior skull base wall suggests the possibility of this disease. Early diagnosis plus comprehensive treatment can improve the survival rate of this disease.
