目的:恶性胸腔科液是晚期肺癌患者常见并发症之一,但目前尚无标准的治疗方案。本研究拟观察HY7000射频热疗机联合胸腔灌注局部化疗治疗恶性胸腔积液的疗效、生存质量和毒副反应。方法:晚期非小细胞肺癌TNMⅢ_B~Ⅳ期合并胸水者共67例,卡氏评分50~60分。按治疗方法随机分为二组,A组34例:射频热疗加胸腔激注羟基喜树碱+白介素-Ⅱ组;B组33例:单纯胸腔灌注羟基喜树碱+白介素-Ⅱ组。热疗2~3次/周,每次60~90分钟,胸腔灌注1次/周,1个疗程3周,观察近期疗效、生存质量及不良反应。结果:A组在控制肺瘤及胸水、改善肺不张、缓解临床症状、生存质量Karnofsky积分的升高及中位生存期延长方面均优于B组,有显著性差异(P<0.05)。结论:射频热疗联合胸腔灌注局部化疗的方法对晚期肺癌合并胸水的治疗效果优于单纯胸腔灌注,近期疗效较理想。
Objective: Malignant pleural fluid is one of the common complications of patients with advanced lung cancer, but there is no standard treatment plan. This study intends to observe the efficacy, quality of life, and toxicity of HY7000 radiofrequency hyperthermia combined with intrathoracic perfusion local chemotherapy for the treatment of malignant pleural effusion. Methods: A total of 67 cases of advanced non-small cell lung cancer with TNM III_B~IV stage pleural effusions were scored from 50 to 60 points. According to the treatment methods were randomly divided into two groups, A group of 34 patients: radiofrequency hyperthermia plus intramuscular injection of hydroxycamptothecin + interleukin-II group; group B 33 cases: simple thoracic infusion of hydroxycamptothecin + interleukin-II group. Hyperthermia 2 to 3 times/week, every 60 to 90 minutes, chest perfusion 1 time/week, 1 course of 3 weeks, observe the short-term efficacy, quality of life and adverse reactions. Results: Group A was superior to group B in controlling lung tumor and pleural effusion, improving lung atelectasis, alleviating clinical symptoms, improving Karnofsky score of survival quality and prolonging median survival. There was a significant difference (P<0.05). Conclusion: The radiofrequency hyperthermia combined with intrathoracic perfusion local chemotherapy is superior to simple thoracic perfusion in the treatment of advanced lung cancer with pleural effusion. The short-term efficacy is ideal.
