目的 :研究新生儿缺氧缺血性脑病 (hypoxic- ischem ic encephalopathy ,HIE)患者血浆α颗粒膜蛋白 -140 (granule m em brane protein- 140 ,GMP- 140 )水平变化 ,探讨其临床意义。方法 :43例 HIE患儿分为轻、中、重 3组 ,并以正常新生儿作为对照。血浆 GMP- 140采用 EL ISA双抗体夹心法测定。结果 :起病初期 ,血浆 GMP- 140水平显著升高 ,并与病情严重度一致。血浆 GMP- 140水平与血小板数量呈负相关 (r =— 0 .44 ,P <0 .0 5 ) ,另轻度 HIE患儿在血小板数量还没有明显下降时 ,GMP- 140就已较对照组显著升高 (P <0 .0 5 ) ,危重儿在恢复期虽然血小板数量明显上升 ,其 GMP- 140水平仍高于对照组 (P <0 .0 5 )。结论 :HIE患儿体内存在血小板活化 ,血浆GMP- 140的变化与病情的严重度一致。监测患儿血浆 GMP- 140的变化 ,有助于血栓前状态的早期诊断及指导治疗 ,并可作为判断预后的指标。
Objective: To study the changes of plasma granulomegalyne-140 (GMP-140) in neonates with hypoxic-ischemic encephalopathy (HIE) and to investigate its clinical significance. Methods: Forty-three HIE children were divided into three groups: light, medium and heavy, with normal newborns as control. Plasma GMP-140 was measured by EL ISA double antibody sandwich method. Results: Early onset, plasma GMP-140 levels were significantly elevated, consistent with the severity of the disease. The level of GMP-140 in plasma was negatively correlated with the number of platelets (r = - 0.44, P <0.05). In addition, when the number of platelets in children with mild HIE had not decreased significantly (P <0.05). Although the number of platelets in critically ill children increased significantly during recovery, the GMP-140 level in critically ill children was still higher than that in control group (P <0.05). Conclusion: There is platelet activation in children with HIE, the change of plasma GMP-140 is consistent with the severity of the disease. Monitoring changes in children with plasma GMP-140, contribute to the early diagnosis and treatment of prethrombotic state, and can be used as an indicator of prognosis.
