目的:探讨烧伤患者急诊切痂植皮处理后采取负压封闭引流(VSD)处理的临床价值。方法:选取我院2012-01-2015-01急诊收治的59例四肢烧伤患者,采用随机数字表法分为VSD组29例和常规组30例,2组患者急诊切痂植皮处理后,常规组予以常规后予以换药和包扎,VSD组予以VSD技术处理,2组患者均给予植皮治疗后对比临床效果。结果:切痂后第1、2、3天,VSD组患者的疼痛程度评分均显著的低于常规组患者且差异均有统计学意义(P<0.05);切痂后第3、7天,VSD组患者的创面细菌阳性率均低于常规组患者且差异均有统计学意义(P<0.05);VSD组患者在切痂后第3、7天的创面细菌阳性率较第1天呈显著的降低趋势(P<0.05);VSD组患者的创面完全愈合时间显著短于常规组患者且差异均有统计学意义(P<0.05);VSD组患者植皮14d后创面愈合率显著的高于常规组(P<0.05)。结论:烧伤患者切痂处理后采取急诊VSD处理有助于缓解患者的疼痛、改善创面清洁度,为后续植皮工作打下良好基础。
Objective: To investigate the clinical value of negative pressure closed drainage (VSD) treatment in patients with burn after emergency eschar excision. Methods: Fifty-nine patients with extremities burns in our hospital from January 2012 to January 2015 were selected and divided into 29 cases of VSD group and 30 cases of conventional group by random number table. After routine dressing and dressing, the VSD group was treated with VSD technique. The two groups of patients were given skin grafting after comparing the clinical effect. Results: The scores of pain in VSD group were significantly lower than those in the control group on day 1, 2 and 3 after escharectomy (P <0.05). On the 3rd and 7th days after escharectomy, The positive rate of wound bacteria in VSD group was lower than that in routine group (P <0.05). The positive rate of wound bacteria in the VSD group on the 3rd and 7th day after escharectomy was significantly higher than that on the 1st day (P <0.05). The complete wound healing time of VSD group was significantly shorter than that of the conventional group (P <0.05). The wound healing rate of the VSD group was significantly higher than that of the conventional group Group (P <0.05). Conclusion: The emergency treatment of VSD after escharectomy can help relieve the patient’s pain, improve the cleanliness of the wound and lay a good foundation for the subsequent skin grafting.
