目的检测胃癌患者血清γ干扰素(IFN-γ)和传统肿瘤标志物的表达,探讨血清IFN-γ与肿瘤标志物的联合监测对胃癌的诊断价值。方法选择83例胃癌患者血清标本(胃癌组),其中女性32例,男性51例;年龄32~74岁,平均年龄60.02岁。同期40例胃良性溃疡患者血清标本(溃疡组),其中女性16例,男性24例;年龄35~68岁,平均年龄58.85岁。40例正常对照者的血清标本(正常对照组),其中女性19例,男性21例;年龄35~65岁,平均年龄58.75岁。应用酶联免疫吸附分析(ELISA)方法检测IFN-γ的表达,并检测肿瘤标志物的表达,比较不同分期胃癌患者IFN-γ和肿瘤标志物的表达。结果胃癌组血清IFN-γ平均浓度(31.71±18.36)μg/L,显著低于溃疡组[(70.78±62.52)μg/L](P=0.000)和正常对照组[(42.92±26.46)μg/L](P=0.039)。IFN-γ与5项肿瘤标志物联合检测可提高灵敏度(65.06%vs34.94%)(P=0.000)和诊断效率(68.71%vs 58.90%)(P=0.007),但降低特异度(72.50%vs 83.75%)(P=0.024)。血清IFN-γ在胃癌各期患者中的灵敏度差异无统计学意义(P=0.958)。结论血清IFN-γ的表达与胃癌的发病密切相关,可作为传统肿瘤标志物的补充,对胃癌的诊断有积极意义。
Objective To detect the expression of serum interferon-γ (IFN-γ) and traditional tumor markers in patients with gastric cancer and to explore the diagnostic value of serum IFN-γ and tumor markers in the diagnosis of gastric cancer. Methods 83 cases of gastric cancer patients (gastric cancer group), including 32 females, 51 males; aged 32 to 74 years, mean age 60.02 years. In the same period, 40 cases of benign gastric ulcer patients (ulcer group), including 16 females, 24 males; aged 35 to 68 years, with an average age of 58.85 years. Serum samples of 40 normal controls (normal control group), including 19 females and 21 males, aged from 35 to 65 years, with an average age of 58.75 years. The expression of IFN-γ was detected by enzyme-linked immunosorbent assay (ELISA), and the expression of tumor markers was detected. The expression of IFN-γ and tumor markers in patients with different stages of gastric cancer were compared. Results The average serum concentration of IFN-γ in gastric cancer group was (31.71 ± 18.36) μg / L, significantly lower than that in ulcer group [(70.78 ± 62.52) μg / L] and normal control group (42.92 ± 26.46 μg / L] (P = 0.039). The combined detection of IFN-γ and 5 tumor markers increased sensitivity (65.06% vs 34.94%) and diagnostic efficiency (68.71% vs 58.90%, P = 0.007), but decreased the specificity (72.50% vs 83.75%) (P = 0.024). There was no significant difference in the sensitivity of serum IFN-γ in all stages of gastric cancer (P = 0.958). Conclusion Serum IFN-γ expression is closely related to the incidence of gastric cancer, which can be used as a complement to traditional tumor markers and has positive significance for the diagnosis of gastric cancer.
