目的分析研究我院10年来内镜诊断的反流性食管炎(RE)病例,评估10年来 RE 发病情况及与相关疾病的关系。方法 1995年1月至2004年12月经胃镜诊断的2088例原发性 RE,按洛杉矶分类进行分级,评估 RE 发病情况,分析患者年龄、性别和伴发疾病的关系。结果原发性 RE 占胃镜检查总人数的4.1%,男性多于女性(4.7%比3.2%,P=0.00).平均年龄55.34岁,男性发病年龄早于女性[(54.37±15.53)岁比(57.40±13.88)岁,P<0.01]。10年间年原发性 RE 检出率和发病年龄上升;随年龄增长 RE 检出率升高,病变程度加重。轻度的(A、B 级)占大多数(82.5%),在 RE 分级上男女差异无统计学意义(P>0.05)。RE 患者 Hp 阳性率低于受检人群(23.5%比42.3%,P<0.01)。Hp阳性 RE 检出率低于阴性(2.9%比4.7%,P<0.01)。Hp 阳性与阴性 RE 在 RE 分级上差异无统计学意义(P>0.05)。结论过去10年间,RE 患病率和发病年龄逐步上升,轻度的占大多数。RE 人群 Hp阳性率低于一般人群。
Objective To analyze the cases of reflux esophagitis (RE) diagnosed by endoscopy in our hospital over the past 10 years and evaluate the relationship between the incidence of RE and its related diseases over the past 10 years. Methods From January 1995 to December 2004, 2088 cases of primary RE diagnosed by gastroscope were graded according to the classification of Los Angeles to assess the incidence of RE and to analyze the relationship between age, gender and concomitant diseases. Results Primary RE accounted for 4.1% of the total number of gastroscopy, more men than women (4.7% vs 3.2%, P = 0.00) .The mean age was 55.34 years old, and the onset age was earlier in males than in females [(54.37 ± 15.53) years vs 57.40 ± 13.88) years old, P <0.01]. In 10 years, the detection rate of primary RE increased and the age of onset increased. With the increase of age, the detection rate of RE increased and the severity of the disease increased. Mild (A, B grade) accounted for the majority (82.5%), in the RE classification, there was no significant difference between men and women (P> 0.05). The positive rate of Hp in RE patients was lower than that of the subjects (23.5% vs. 42.3%, P <0.01). The positive rate of Hp-positive RE was lower than negative (2.9% vs 4.7%, P <0.01). Hp positive and negative RE in the RE classification difference was not statistically significant (P> 0.05). Conclusions The prevalence of RE and age of onset have gradually risen over the past 10 years with a mild majority. The positive rate of Hp in RE population is lower than that in the general population.
