目的探讨同型半胱氨酸(Hcy)、糖化血红蛋白(HbA1c)和尿微量白蛋白(U-mAlb)联合检测对糖尿病早期肾脏损伤的临床价值。方法选择天津市河西区马场街社区卫生服务中心2015年1月-2016年1月就诊的早期糖尿病肾脏损伤患者92例作为观察组,选择同期健康体检者90例作为对照组,早晨空腹抽取静脉血3 ml,分离血清进行Hcy、HbA1c、U-mAlb水平检测,对检测结果进行比较分析。结果观察组Hcy、HbAlc、U-mAlb水平均高于对照组,差异有统计学意义(P<0.05);观察组Hcy、HbAlc、U-mAlb异常检出率均高于对照组,差异有统计学意义(P<0.05);Hcy、HbAlc、U-mAlb联合检测的诊断阳性率均高于其他检测方法,差异有统计学意义(P<0.05)。结论在早期糖尿病肾脏损伤诊断中,采取Hcy、HbA1c、U-mAlb联合检测的检出率较高,具有良好的临床价值。
Objective To investigate the clinical value of combined detection of homocysteine (Hcy), glycosylated hemoglobin (HbA1c) and urinary microalbumin (U-mAlb) on early diabetic nephropathy. Methods Ninety-two patients with early diabetic kidney injury from January 2015 to January 2016 in Madi Street Community Health Service Center, Hexi District, Tianjin were selected as the observation group. Ninety healthy subjects were selected as the control group in the morning, 3 ml, the serum was separated for Hcy, HbA1c, U-mAlb level test, the test results were compared. Results The levels of Hcy, HbAlc and U-mAlb in the observation group were significantly higher than those in the control group (P <0.05). The detection rates of Hcy, HbAlc and U-mAlb in the observation group were higher than those in the control group (P <0.05). The positive rates of Hcy, HbAlc and U-mAlb were higher than those of other methods (P <0.05). Conclusion In the early diagnosis of diabetic nephropathy, the combined detection of Hcy, HbA1c and U-mAlb has a high detection rate and has good clinical value.
