目的探讨胃癌患者围术期血清IL-2、IL-6、TNF-α、NO及IGF-Ⅱ水平变化及临床意义。方法以35例胃癌患者作为观察组,30例正常人为对照组,监测围术期观察组IL-2、IL-6、TNF-α、NO和IGF-Ⅱ水平变化并分析。结果观察组术前IL-2、NO水平均显著低于对照组,IL-6、TNF-α、IGF-Ⅱ水平则显著高于对照组,组间比较差异均有统计学意义(P﹤0.05);观察组IL-2、NO术后d1均轻度升高,但与术前比较差异无统计学意义(P﹥0.05),术后d3、d7、d10时IL-2、NO水平逐渐升高,与术前比较差异均有统计学意义(P﹤0.05),但d10时IL-2水平仍显著低于对照组,组间比较差异有统计学意义(P﹤0.05),而NO水平则接近对照组,组间比较差异无统计学意义(P﹥0.05);观察组IL-6、TNF-α、IGF-Ⅱ术后d1均轻度降低,但与术前比较差异无统计学意义(P﹥0.05),术后d3、d7、d10IL-6、TNF-α、IGF-Ⅱ水平逐渐降低,与术前比较差异均有统计学意义(P﹤0.05),d10时均已接近对照组水平,组间比较差异无统计学意义(P﹥0.05)。结论胃癌患者血清中存在多种细胞因子分泌调节紊乱,监测围术期血清中IL-2、IL-6、TNF-α、NO及IGF-Ⅱ水平的变化对胃癌诊断、病情判断及预后估计有一定的参考价值。
Objective To investigate the changes of perioperative serum levels of IL-2, IL-6, TNF-α, NO and IGF-Ⅱ in patients with gastric cancer and their clinical significance. Methods 35 cases of gastric cancer patients as observation group and 30 normal controls as control group. The changes of IL-2, IL-6, TNF-α, NO and IGF-Ⅱ in perioperative period were observed and analyzed. Results The levels of IL-2 and NO in the observation group before operation were significantly lower than those in the control group. The levels of IL-6, TNF-α and IGF-Ⅱ in the observation group were significantly higher than those in the control group (P <0.05 ). The levels of IL-2 and NO in the observation group increased slightly after operation, but there was no significant difference between the two groups (P> 0.05). The levels of IL-2 and NO in the observation group gradually increased after d3, d7 and d10 (P <0.05). However, the level of IL-2 at d10 was still significantly lower than that of the control group (P <0.05), while the level of NO was significantly higher than that of the control (P> 0.05). The d1 of IL-6, TNF-α and IGF-Ⅱ in the observation group decreased slightly after operation, but there was no significant difference compared with the preoperative (P <0.05). The levels of d3, d7, d10IL-6, TNF-α and IGF-Ⅱ in postoperative patients were lower than those in control group , There was no significant difference between the two groups (P> 0.05). Conclusion There are many disorders in the regulation of the secretion of cytokines in the serum of patients with gastric cancer. The changes of serum IL-2, IL-6, TNF-α, NO and IGF-Ⅱ in perioperative period are related to the diagnosis, prognosis and prognosis of gastric cancer Certain reference value.
