我科自1986年以来,采用髂骨整复鞍鼻12例,效果满意,介召如下。 1 临床资料 12例患者中,男8例,女4例,年龄21～42岁。3例为先天性,6例由萎缩性鼻炎引起,2例发生于鼻中隔脓肿后,1例鞍鼻整复后又塌陷。 2 手术方法 2.1 髂骨充填物制备 2.1.1 取髂骨 术前3天,髂骨每日消毒备皮一次。局麻,沿髂骨的嵴做切口,分离髂骨骨膜,用骨凿截取长约5cm,宽约2cm一段髂骨嵴备用,逐层缝合切口。 2.1.2 充填物制备 先将取下的髂骨初雕成“L”形状。“L”的长边为体部,相当于鼻根到鼻尖的长度,约5cm。“L”的短边为支柱,相当于鼻小柱的高度,约1cm。在“L”形基础上再按需要细雕。将体部刻成窄槽,以便固定于鼻梁部,体部两边刻薄,厚度根据鞍鼻凹陷的深浅而定。体部
