目的:研究不同剂量固尔苏治疗NRDS的疗效。方法:回顾性分析71例2006年1月~2009年3月出生曾用固尔苏的早产儿,按固尔苏使用剂量分成大剂量组(160~220 mg/kg)和小剂量组(80~120 mg/kg)。观察治疗前后症状体征变化,统计治疗前后氧疗时间、通气时间等指标,采用t检验进行比较。结果:患儿治疗后症状及体征明显改善,治疗后的吸入氧浓度与治疗前比较明显下降(P<0.05)。与小剂量组比较,大剂量组所需氧浓度、需辅助通气时间、氧疗时间均较少。两组患儿对PEEP、PIP的需求及需机械通气比例相近。两组相比慢性肺疾病、动脉导管未闭、脑室内出血发生率、时间以及新生儿死亡率间的差异均无统计学意义(P>0.05)。结论:固尔苏运用可明显改善患儿症状体征,显著减低需氧量。使用较大剂量固尔苏有缩短辅助通气时间的趋势。
Objective: To study the curative effect of different dosages of cortisol on NRDS. Methods: A retrospective analysis of 71 premature infants who had been born with celiac from January 2006 to March 2009 was retrospectively analyzed. The patients were divided into two groups according to the dose of cortezol (160-220 mg / kg) and the low dose group ~ 120 mg / kg). Before and after treatment to observe changes in symptoms and signs, statistics before and after treatment oxygen therapy time, ventilation time and other indicators, using t test for comparison. Results: After treatment, the symptoms and signs of infants were significantly improved. The inhaled oxygen concentration decreased significantly after treatment (P <0.05). Compared with the low-dose group, high-dose oxygen concentration required for auxiliary ventilation time, oxygen therapy time are less. Two groups of children on the PEEP, PIP demand and the need for mechanical ventilation similar proportions. There were no significant differences between the two groups in chronic lung disease, patent ductus arteriosus, incidence of intraventricular hemorrhage, time and neonatal mortality (P> 0.05). Conclusion: The use of Gultsu could significantly improve children’s symptoms and signs and significantly reduce the oxygen demand. The use of larger doses of cefalta has a tendency to shorten the duration of assisted ventilation.
