目的:探讨长治市幽门螺杆菌抗体在体检人群中的流行病学特征。方法:选取2019年1月至2019年12月长治市人民医院消化内科体检的1 224名健康志愿者的临床资料,男852名,女372名,年龄(48.03±17.07)岁,年龄范围为16~96岁,采用蛋白芯片技术行幽门螺杆菌抗体谱细胞毒素相关蛋白A(CagA)、空泡毒素相关蛋白(VacA)、尿素酶检测,观察志愿者幽门螺杆菌抗体分布情况。结果:志愿者幽门螺杆菌尿素酶抗体阳性率为65.0%(795/1 224)。女性志愿者的尿素酶抗体阳性率[69.6%(259/372)]高于男性[62.9%(536/852)],差异有统计学意义(n P<0.05)。20~29岁女性志愿者尿素酶抗体阳性率[72.3%(47/65)]高于男性志愿者[53.0%(35/66)],差异有统计学意义(n P<0.05)。在幽门螺杆菌感染者中,Ⅰ型幽门螺杆菌毒力因子感染占67.8%(539/795),Ⅱ型幽门螺杆菌毒力因子感染占32.2%(256/795)。男性尿素酶抗体阳性的志愿者Ⅰ型348名,Ⅱ型188名;女性尿素酶抗体阳性的志愿者Ⅰ型191名,Ⅱ型68名,不同性别间的幽门螺杆菌毒力因子分型分布比较,差异有统计学意义(n P<0.05)。20~29岁男性志愿者幽门螺杆菌毒力因子分型的分布(Ⅰ型22名,Ⅱ型13名)与同年龄女性志愿者(Ⅰ型39名,Ⅱ型8名)比较,差异有统计学意义(n P<0.05)。n 结论:长治市地区体检人群幽门螺杆菌感染率较高;20~29岁女性尿素酶抗体阳性率高于男性,以Ⅰ型感染为主。“,”Objective:To investigate the epidemiological characteristics of helicobacter pylori(HP)antibodies among the physical examination population in Changzhi.Methods:In this cross-sectional study, 1 224 people who took physical examination in Changzhi People′s Hospital from January 2019 to December 2019 were recruited into this study, including 852 males and 372 females.The mean age of the study population was(48.03±17.07)years old, ranging 16 to 96 years old.The protein chip technology was used to detect the HP antibody profile including cytotoxin associated protein A(CagA), vacuolating cytotoxin A(VacA), Ure IgG, to observe helicobacter pylori antibody distribution of volunteers.Results:The positive rate of HP antibody among volunteers was 65.0%(795/1 224). The positive rate of Ure IgG in female[69.6%(259/372)]was significantly higher than that in male[62.9%(536/852)], and the difference was statistically significant(n P<0.05). Among the 20 to 29 years old population, the positive rate of Ure IgG in females[72.3%(47/65)]was significantly higher than that in males[53.0%(35/66)], and the difference was statistically significant(n P<0.05). In HP infected population, 67.8%(539/795)of them were infected with typeⅠ HP virulence factor, while 32.2%(256/795)were infected with type Ⅱ HP virulence factor.There were 348 males volunteers with typeⅠ positive rate of Ure IgG and 188 male volunteers with typeⅡ positive rate of Ure IgG, 191 females volunteers with typeⅠ and 68 females volunteers with typeⅡ, the genotyping distribution of virulence factors of HP between different genders was compared, and the difference was statistically significant(n P<0.05). The distribution of HP virulence factor typing in male volunteers aged 20 to 29 years old(22 cases of typeⅠ and 13 cases of typeⅡ)was significantly different from that of females volunteers of the same age(39 cases of typeⅠ and 8 cases of type Ⅱ, and the difference was statistically significant(n P<0.05).n Conclusion:The infection rate of HP is higher in the physical examination population in Changzhi city.The positive rate of Ure IgG in females aged 20 to 29 years was higher than that in males, and the main infection is typeⅠ.
