目的通过测定常规PTCA组及缺血预适应处理组术前、术后患者肌钙蛋白Ⅰ(CTnI)、C-反应蛋白(CRP)与混合静脉血氧饱和度(SvO2)的变化,了解两种PTCA方法对冠脉病变患者的心肌细胞、心功能的影响。方法首先设定缺血预适应(IP)方案,观察IP组患者51例、常规PTCA组(UC组)患者53例术后CRP、CTnⅠ、SvO2的变化以及术中不良事件发生与否。结果IP组术前、术后C-反应蛋白(CRP)、肌钙蛋白I(CTnI)、混合静脉血氧饱和度(SvO2)的变化程度低于UC组,差异有统计学意义(P<0.05)。IP组术中发生心绞痛及ECGST段缺血型改变的几率低于常规PTCA组,差异有统计学意义(P<0.05)。结论缺血预适应方案可以作为PTCA操作的常规方法,它可以减轻介入操作对心肌细胞的损伤,降低冠脉介入术中、术后的不良事件的发生。
Objective To investigate the changes of CTnI, C-reactive protein (CRP) and mixed venous oxygen saturation (SvO2) in patients with conventional PTCA and ischemic preconditioning Effect of PTCA on Myocardial Cells and Cardiac Function in Patients with Coronary Lesions. Methods The ischemic preconditioning (IP) protocol was set up. The changes of postoperative CRP, CTn Ⅰ and SvO 2 in 51 IP patients and 53 patients with conventional PTCA (UC group) were observed and the occurrence of intraoperative adverse events were observed. Results The changes of preoperative and postoperative C - reactive protein (CRP), troponin I (CTnI) and mixed venous oxygen saturation (SvO2) in IP group were significantly lower than those in UC group (P <0.05 ). The incidence of angina pectoris and ischemic ECGST segment in IP group was lower than that in routine PTCA group (P <0.05). Conclusions The ischemic preconditioning regimen can be used as a routine method for PTCA. It can reduce the damage of myocardial cells during intervention and reduce the occurrence of adverse events during and after coronary intervention.
