桥本脑病(HE)是一种目前尚存在争议的神经系统功能紊乱,包括患者出现的一组异质性的神经系统症状以及抗甲状腺抗体的水平增高。HE的临床表现可包括脑病的特点,如抽搐、精神行为异常、运动障碍和昏迷。尽管被认为与桥本甲状腺炎或甲状腺功能紊乱相关,HE最常见的免疫学异常是出现血清抗甲状腺球蛋白抗体(TGAb)或抗甲状腺过氧化物酶(antimicrosomal)抗体(TMAb)的增高。目前尚不清楚抗甲状腺抗体的增高是继发于脑病的一种偶发免疫异常还是其确实导致了疾病的发生。HE对激素或者血浆置换等治疗的有效反应也支持这种属于免疫异常发病机制的推论。
Hashimoto’s encephalopathy (HE) is a currently debatable neurological disorder characterized by a heterogeneous set of neurological symptoms and increased anti-thyroid antibodies in patients. The clinical manifestations of HE may include features of encephalopathy such as convulsions, mental disorders, dyskinesia, and coma. Although considered to be associated with Hashimoto’s thyroiditis or thyroid dysfunction, the most common immunological abnormalities of HE are the presence of elevated serum anti-thyroglobulin antibody (TGAb) or anti-thyroid antimicrosomal antibody (TMAb). It is not yet clear whether an elevated thyroid antibody is an incidental immune disorder secondary to encephalopathy or does it cause the disease. The effective response of HE to the treatment of hormones or plasmapheresis also supports this corollary of the pathogenesis of immune abnormalities.
