The purpose is to investigate the effects of flight environment on the activity and complement level of natural killer cells (NKCs) in peripheral blood of pilots. Methods Nine ground crew members, the average age of 36 ± 7 7 years old; pilots 78, the average age of 36 ± 60 years. Morning fasting by the elbow venous blood 10ml, were placed in heparin anticoagulant tubes and non-anticoagulant tubes of 5ml for inspection, NKC activity using tritiated-thymidine incorporation (3H-TdR) incorporation assay, serum complement C3, C4 using rapid immunodialysis determination. Results Compared with ground crew, the levels of NKC activity and serum complement in peripheral blood of pilots were lower, and the levels of serum C3 and C4 were significantly decreased. Comparisons within the flight group show the most significant decreases in NKC activity and serum complement levels in pilots and bomber pilots. Conclusions Factors such as ionizing radiation in the flight environment may be an important reason for reducing NKC activity and serum complement levels in pilots’ peripheral blood
