[目的]探索泰兴市幽门螺杆菌(Hp)与其它危险因素致胃癌的交互作用。[方法]选择92例胃癌新发病例按性别、年龄(±2岁)和居住地进行1∶1配对的病例对照研究 ,对全部研究对象检测了Hp抗体。[结果]病例组Hp阳性率(54.35%)显著高于对照(33.70%),Hp阳性患胃癌的OR值为2.34(95%CI1.23~4.48) ,Hp阳性同时又暴露于长期吃新鲜蔬菜少、长期吃新鲜水果少、不吃香菇和慢性胃炎因素致胃癌发生的OR值分别为5.34~5.39,4.11~6.72,4.72和6.50,其两因素交互作用指数分别为0.34~0.42,0.24~0.67,0.44和0.01,Hp阳性合并上述危险因素可能存在相加的交互作用方式 ,在控制了彼此的混杂作用后 ,下列因素致胃癌的作用大小依次为 :胃溃疡(OR=18.0)>Hp阳性合并胃炎(OR=9.00)>慢性胃炎(OR=6.38)>Hp阳性(OR=3.43)。[结论]Hp感染是泰兴市胃癌高发的重要危险因素之一 ,其与上述危险因素在致胃癌的关系中存在着显著的协同作用。
[Objective] To explore the interaction between Helicobacter pylori (Ta) and gastric cancer caused by other risk factors in Taixing. [Method] A total of 92 cases of newly diagnosed cases of gastric cancer were selected as the case-control study of 1: 1 pairing by sex, age (± 2 years) and place of residence. Hp antibodies were detected in all the subjects. [Results] The positive rate of Hp in case group was significantly higher than that in control group (54.35%, 33.70%). The OR of Hp positive gastric cancer was 2.34 (95% CI 1.23-4.48). Hp positive was also exposed to long-term eating fresh vegetables The OR values of less, long-term eating less fresh fruit, not eating mushrooms and chronic gastritis were 5.34-5.39, 4.11-6.72, 4.72 and 6.50, respectively, and the two-factor interaction index were 0.34-0.42 and 0.24-0.67 , 0.44 and 0.01, Hp positive merger risk factors may exist additive interaction mode, after controlling each other’s confounding effect, the following factors cause gastric cancer in the order of size: gastric ulcer (OR = 18.0)> Hp positive merger Gastritis (OR = 9.00)> Chronic gastritis (OR = 6.38)> Hp-positive (OR = 3.43). [Conclusion] Hp infection is one of the most important risk factors for gastric cancer in Taixing. There is a significant synergistic effect between Hp infection and the above risk factors in gastric cancer.
