慢粒白血病(CML)和特发性骨髓纤维化(IMF)同属于骨髓增生性疾病。这些疾病的骨髓象通常表现为三个细胞系统发生不同程度的改变,但亦可表现为一个细胞系统的改变、其特征为各种成份不成比例地增加或减少,或成熟障碍。除了细胞增生外,骨髓尚发生对本病具有诊断价值的基质改变,如骨髓纤维化等。外周血不一定能反映出骨髓中各种成份的变化情况。骨髓细胞涂片检查亦不能反映以上的改变情况。因此单纯根据临床表现和骨髓细胞涂片检查的结果对CML病史不明显的慢粒急变和急粒白血病,以及CML和IMF等疾病进行鉴别诊断有时会发生困难。为了探讨解决以上的诊断问题,本文作者对12例CML和13例IMF患者连
CML and idiopathic myelofibrosis are both myeloproliferative diseases. Bone marrow signs of these disorders often manifest themselves to varying degrees in three cellular systems, but can also manifest as changes in a cellular system characterized by disproportionate increases or decreases in various components, or maturation disorders. In addition to cell proliferation, the bone marrow is still occurring on the diagnosis of the disease with the matrix changes, such as bone marrow fibrosis. Peripheral blood may not necessarily reflect the changes in the various components of the bone marrow. Bone marrow smear examination can not reflect the above changes. Therefore, based on clinical manifestations and bone marrow smear test results of CML obvious history of CML and acute myeloid leukemia, as well as CML and IMF and other diseases differential diagnosis sometimes difficult.
