目的:探讨单抗OC859治疗对卵巢癌病人免疫系统及生存率的影响。方法:对曾用过单抗OC859治疗137例(单抗组)及未用过单抗的154例(对照组)卵巢癌病人进行回顾性分析,测定单抗组部分患者血清中的人抗鼠抗体(HAMA)(30例) 和T细胞亚群(21例)的改变,并比较两组病人的生存率。结果:两组年龄、满意的肿瘤细胞减灭术比例、病理类型、分期、分化差异均无显著性。单抗治疗后HAMA滴度明显升高。在单抗治疗之前病人外周血中CD4/CD8比值低于正常水平,而单抗治疗一次后CD4/CD8比值大部分达正常水平,单抗治疗两次后CD4/CD8比值全部达正常水平。单抗组平均生存时间、3年及5年生存率分别为91±6月、66.1和62.3%,对照组平均生存时间、3年及5年生存率分别为61±6月、41.4%和36.7%,差异均有极显著性(P均<0.001)。结论:单抗OC859治疗使卵巢癌病人的细胞免疫和体液免疫系统均有明显改变,3年及5年生存率均明显提高,在手术和化疗的基础上单抗OC859有助于卵巢癌的治疗。
Objective: To investigate the effect of monoclonal antibody OC859 on the immune system and survival rate of patients with ovarian cancer. Methods: A retrospective analysis of 137 ovarian cancer patients who had been treated with monoclonal antibody McAb OC859 (monoclonal antibody group) and 154 non-used monoclonal antibody (control group) was performed. Serum anti-mouse Antibody (HAMA) (30 cases) and T cell subsets (21 cases), and to compare the survival rate of two groups of patients. Results: There was no significant difference between the two groups in the proportion of tumor cytoreductive surgery, pathological type, stage and differentiation. HAMA titer was significantly increased after MAb treatment. The CD4 / CD8 ratio in peripheral blood of patients before the monoclonal antibody treatment was lower than the normal level. However, the ratio of CD4 / CD8 was mostly normal after the monoclonal antibody treatment. The ratio of CD4 / CD8 reached the normal level after the monoclonal antibody treatment twice. The mean survival time, 3-year and 5-year survival rates of the monoclonal antibody group were 91 ± 6 months, 66.1% and 62.3% respectively. The mean survival time, 3-year and 5-year survival rates of the control group were 61 ± 6 months, 41.4% and 36.7% %, The differences were significant (P <0.001). Conclusion: The monoclonal antibody OC859 treatment significantly changes the cellular immunity and humoral immune system in patients with ovarian cancer, and the 3-year and 5-year survival rates are significantly improved. On the basis of surgery and chemotherapy, monoclonal antibody OC859 contributes to the treatment of ovarian cancer .
