目的为探讨唐山市射线接触者血像变化的有关影响因素。方法对唐山市552例从业人员进行了血像检查及有关因素调查。结果逐步回归分析发现年龄、工龄、射线种类、累积受照剂量是影响射线工作者WBC的主要因素;微核、双着丝粒染色体的发生及肝功异常是Pt改变的关联因素;性别差异是Hb改变的主要因素。针对各影响因素进行分层分析发现,大部分血像虽波动在正常值范围内,但均低于正常平均值,说明射线工作者血像有下降趋势。女性红细胞系及白细胞系对射线作用较男性敏感。工龄15年以上组血像有下降趋势;累积剂量达20mGy以上组WBC有下降趋势;X射线较其他射线对接触者影响小;HBsAg阳性者血像较低。结论提示射线接触者血像的改变是多因素共同作用的结果。
Objective To explore the influencing factors of the blood picture changes of radiation contact persons in Tangshan City. Methods A total of 552 practitioners in Tangshan City were surveyed for hemogram and related factors. The results of stepwise regression analysis found that age, length of service, types of radiation, cumulative exposure dose is the main factor affecting the WBC workers; micronuclei, dicentric chromosomes and abnormal liver function Pt is associated with changes in factors; sex difference is Hb change the main factors. According to the stratified analysis of various influencing factors, most of the blood samples fluctuated within the normal range, but both were lower than the normal average value, indicating that the blood worker’s radiogram had a downward trend. Female erythroid and leucocyte lines are more sensitive to men than men. Over 15 years of service, the blood picture had a declining trend; WBC with a cumulative dose of more than 20mGy had a decreasing trend; X-ray had less influence on the contact than other radiographs; and the blood picture of HBsAg-positive people was lower. The conclusion suggests that the change of the blood picture of the radiation contact is the result of multi-factor cooperation.
