[目的]通过对辽宁省2006~2008年癌症临床相关信息的分析,论证建立辽宁省癌症临床信息网络传输系统的可行性和目前癌症临床主要信息产生质量的现状及问题。[方法]采用整群二阶段随机抽样,抽取不同类型医疗机构外科、化疗科(或内科)、放疗科常见癌症的病志,填写调查表,按照治疗方式、诊治规范情况、TNM分期、病案首页填写情况、化疗KPS评分情况、放疗疗效评价信息等进行频数分布的描述及卡方检验。[结果]共收集有效调查表4514张,所有癌症的治疗方式主要以手术和化疗为主,构成比分别为43.13%和40.16%。手术、化疗和放疗规范诊治率分别为33.38%、51.08%和37.27%。手术、化疗和放疗患者TNM分期的书写率分别为55.42%、49.48%和41.78%。病案首页缺项率为15.28%,外科为18.28%,化疗为10.20%,放疗为19.76%。化疗患者有入院KPS评分记录为31.77%,出院有KPS评分记录为23.99%。放疗结束时效果评价有记录为44.96%;放疗结束时正常组织损伤评价有记录为37.27%。早期癌症患者为8.97%。[结论]建立辽宁省癌症临床信息网络传输系统是可行的,但网络传输的主要信息条的质量有待提高。
[Objective] Based on the analysis of cancer clinical information in Liaoning Province from 2006 to 2008, the feasibility and feasibility of setting up Liaoning Cancer Clinical Information Network Transmission System and the present status and problems of the current clinical quality of cancer clinical information were demonstrated. [Methods] A total of two groups of random sampling were adopted in this study. [Results] A total of 4514 valid questionnaires were collected. The treatment of all cancers was mainly surgery and chemotherapy, with the proportions of 43.13% and 40.16% respectively. The standard diagnosis and treatment rates of surgery, chemotherapy and radiotherapy were 33.38%, 51.08% and 37.27% respectively. The rates of TNM staging in surgery, chemotherapy and radiotherapy were 55.42%, 49.48% and 41.78% respectively. Home page missing item rate of 15.28%, surgery was 18.28%, chemotherapy was 10.20%, radiation was 19.76%. Chemotherapy patients admitted admission KPS score was recorded as 31.77%, discharged KPS score recorded as 23.99%. The end of radiotherapy was recorded as 44.96%, and at the end of radiotherapy, normal tissue damage was recorded as 37.27%. Early cancer patients 8.97%. [Conclusion] It is feasible to establish Liaoning Cancer Clinical Information Network Transmission System, but the quality of the main information transmission network needs to be improved.
