[目的]探讨行走年龄发育性髋关节脱位患儿术后发生股骨骨折的原因及预防措施。[方法]回顾性分析2008年5月~2012年1月本科收治的86例接受手术治疗的发育性髋关节脱位患儿,5例术后发生患侧股骨低暴力性骨折,其中男1例,女4例,接受手术治疗时患儿年龄为2~6岁(平均3岁5个月)。结合临床深入探讨术后发生股骨骨折的原因,并采用髓内针固定或石膏外固定等进行治疗。[结果]所有患儿均获得随访,时间12~27个月,平均17.6个月。骨折愈合良好。均恢复下肢行走和负重功能。[结论]切开复位手术造成的损伤、长时间的石膏外固定、内固定器械的使用等是造成发育性髋脱位患儿术后股骨骨折的主要原因,术中操作规范,术后循序渐进的进行功能锻炼,避免过早负重等措施是减少或预防术后股骨骨折的重要措施。
[Objective] To explore the causes of postoperative femoral fractures in pediatric patients with developing hip dislocation and preventive measures. [Methods] A retrospective analysis of 86 cases undergoing surgical treatment of developmental hip dislocation in our hospital from May 2008 to January 2012 was retrospectively analyzed. Five cases suffered from ipsilateral femoral fractures after operation, including 1 males, 4 females, children undergoing surgical treatment of children aged 2 to 6 years (mean 3 years 5 months). Combined with clinical in-depth discussion of the reasons for the occurrence of femoral fractures, and the use of intramedullary nail fixation or plaster treatment. [Results] All patients were followed up for 12-27 months with an average of 17.6 months. Fracture healing well. All recovered lower limb walking and weight-bearing functions. [Conclusion] The main causes of postoperative femoral fractures in children with developmental dislocation of hip are the injuries caused by open reduction surgery, prolonged gypsum external fixation and the use of internal fixation devices. The operation rules and procedures are gradually followed up Functional exercise, to avoid premature weight and other measures to reduce or prevent postoperative femoral fractures important measures.
