目的探讨声触诊组织量化(VTQ)技术在乳腺良、恶性肿块鉴别诊断中的应用价值。方法有乳腺肿块女性患者107例(123个肿块)。其中,良性肿块76个(A组),恶性肿块47个(B组)。采用VTQ技术分别测量并比较两组乳腺肿块内部、边缘及正常乳腺腺体的剪切波速度(SWV)。结果 A组肿块内部、边缘和正常乳腺腺体SWV均低于B组(P<0.01)。取SWV 3.795m/s为最佳临界值,VTQ技术诊断乳腺肿块的灵敏度为80.85%,特异度为94.74%,与病理诊断的一致率为89.43%。结论 VTQ技术作为常规超声的有效补充,在乳腺良、恶性肿块的鉴别诊断方面有较高的临床应用价值。
Objective To explore the application value of acoustic palpation Tissue Quantification (VTQ) in the differential diagnosis of benign and malignant breast tumors. Methods Female patients with breast lumps of 107 cases (123 lumps). There were 76 benign masses (group A) and 47 malignant masses (group B). The shear wave velocity (SWV) of the internal, marginal and normal mammary glands in the two groups of breast masses was measured and compared using the VTQ technique. Results The SWVs in the mammary gland of the internal, marginal and normal mammary glands in group A were lower than those in group B (P <0.01). The SWV 3.795m / s was the best cutoff value. The sensitivity of VTQ technique in diagnosing breast masses was 80.85% and the specificity was 94.74%. The coincidence rate with pathological diagnosis was 89.43%. Conclusion As an effective supplement of routine ultrasound, VTQ technique has high clinical value in the differential diagnosis of benign and malignant breast tumors.
