目的评价多层螺旋CT血管成像(multi-slice computed tomography angiography,MSCTA)在诊断颅外颌面部动静脉畸形(arteriovenous malformation,AVM)中的价值。资料与方法对临床怀疑为颅外颌面部AVM的48例患者行MSCTA检查,扫描后通过容积再现(VR)、最大密度投影(MIP)处理以分析该病变的特殊表现。46例行数字减影血管造影(DSA)检查,17例行手术切除。结果 48例中,MSCTA显示畸形血管团者48例(100%);显示供血动脉者43例(89.6%);显示引流静脉者44例(91.7%)。13例(27.1%)软组织AVM合并有颅颌面骨异常(4例颌骨;9例其他颅面骨)。结论多数颅外颌面部AVM的表现特点能在MSCTA上清楚显示。作为一种无创检查方法,MSCTA能为临床诊断和治疗提供有益信息。
Objective To evaluate the value of multi-slice computed tomography angiography (MSCTA) in the diagnosis of arteriovenous malformation (AVM) in the extracranial maxillofacial region. MATERIALS AND METHODS: Forty-eight patients with clinical suspicion of extracranial maxillofacial AVM underwent MSCTA. After scanning, volume rendering (VR) and maximum density projection (MIP) were performed to analyze the special features of this lesion. 46 routine digital subtraction angiography (DSA) examination, 17 cases underwent surgical resection. Results In 48 cases, MSCTA showed 48 cases (100%) of abnormal blood vessels, 43 cases (89.6%) showed arterial blood supply, and 44 cases (91.7%) showed drainage veins. Thirteen (27.1%) soft-tissue AVMs had craniofacial anomalies (4 jaws; 9 other craniofacial bones). Conclusion The features of AVM in most cranial and maxillofacial regions can be clearly shown on MSCTA. As a non-invasive method, MSCTA can provide useful information for clinical diagnosis and treatment.
